a. Interprets medication orders (verbal and written) and accurately enters such information into the patient's medication profile
b. Assures adequate control and documentation of controlled substances
c. Assists in the annual inventory and biannual controlled substance inventory in accordance with state and federal regulations
d. Accurately dispenses medications to employees as evidenced by lack of occurrence reports and/or complaints
2. Demonstrates competence in the performance and responsibilities as they relate to the review of medication orders assuring they will contribute to positive patient outcomes
. Accurately monitors drug therapy regimens for contraindications, drug/drug interactions, food/drug interactions, allergies and appropriateness of drug dose and frequency
a. Provides clinical consultation and clarification to practitioners and patients. Suggests appropriate, cost effective therapeutic alternatives to medical staff as needed, as evidenced by clinical intervention logs.
3. Contributes to the quality and effective operation of the department.
. Answers the telephone in a courteous and timely manner and identifies self by name when taking calls. Responds or redirects requests as appropriate, as evidenced by personal monitoring and number of complaints received.
a. Participates in department quality programs as evidenced by documentation of meeting attendance and reports produced.
b. Works independently with minimal supervision. Organizes and prioritizes work assignments. Ensures pharmacy services are provided in a timely manner as evidenced by lack of complaints.
4. Demonstrates proficiency in information systems including clinical information system, automated dispensing system, and vault management system.
. Knowledgeable on those clinical modules necessary to provide appropriate patient care as evidenced by lack of error reports.
a. Able to implement or describe backup plan in event of power or communications failure in accordance with established procedure.
b. Able to do minimal troubleshooting of information systems
- Over three months current pharmacy experience
Experience Preferred/Desired
- Over one year pharmacy experience
- Health System outpatient experience
Education Minimum Required
- Graduate of an ACPE accredited School of Pharmacy with a BS in Pharmacy
Education Preferred/Desired
- Pharm.D
Special Skills Minimum Required
- Ability to work independently with minimal direct supervision.
- Ability to work cooperatively with health-system and pharmacy staff.
- Ability to handle frequent interruptions and adapt to changes in workload and work schedule.
- Ability to set priorities, make critical decisions and respond quickly to emergency requests.
Licensure and/or Certifications Required
- NH License, Registered Pharmacist
