Job Summary and Qualifications

Position Responsibilities:

1. Responsible for the provision of comprehensive clinical pharmacy services for the ED and all associated areas (eg. Pediatrics, Trauma, Urgent Care) including pharmacokinetic and therapeutic consultation.

2. Provide Pharmacy review of medication orders prior to administration and ensure safe and effective drug therapy for ED patients.

3. In coordination with Clinical Coordinator, prepare formulary documents for new Emergency Department requests, protocols, policies, and order sets and make recommendations regarding place on formulary. Develop guidelines to ensure appropriate use of high-cost and/or specialized drugs used in the ED.

4. Participate in the development of medication use management programs within this facility including clinical guidelines, critical pathways, disease management, and drug use programs.

5. Acts as a preceptor to students and/or residents when applicable.

6. Participate in the professional development and competency of clinical staff. Functions as a role model and as an educational resource for pharmacy staff.

    • Serve as a resource to the Emergency Department. Provide inservices and lectures to medical staff and other healthcare providers when applicable.

8. Keeps informed of all local, state and federal laws covering the storage, handling and dispensing of drugs and for interpreting each prescription order to determine that it meets all legal requirements.

9. Keeps informed of the action, side effects and proper usage of all new drugs as they are made commercially available as well as of all investigational drugs being studied at this institution.

10. Maintains awareness of contemporary trends in the profession through the professional literature and regular attendance at professional organization meetings, institutes and seminars.

11. Provides patient specific medication teaching for discharge medications when appropriate.

12. Focus on cost avoidance and cost savings due to medication use in the Emergency Department.

13. Assist with medication reconciliation process for patients admitted to facility through the ED.

14. Assist in obtaining medication allergies and reactions from ED patients.

15. Proficient in the use of Microsoft Office applications, Meditech or other required operating systems, Order Scanning Systems, Automated Dispensing Units, Packaging Equipment, Physician Order Entry, and Clinical Integration to systems.

16. Maintain effective communication with Skyline directors and leadership teams on emergency department management opportunities

17. Supervise and direct support Personnel

18. Work with Staff and Clinical Pharmacists with dosing and coverage of all patients in the hospital when called upon.

19. Work with Quality and Pharmacy on Medication reconciliation, Stroke, SCIP, and all Core measures.

  1. Participates in special projects as needed and performs other duties as assigned.

Department Specific Criteria:

  • Performs assigned department based monitoring duties in a timely manner.
  • Directs activities of work area effectively and establishes priorities.
  • Gives a thorough, concise report at change of shift, within allotted time. Communicates incomplete assignments, and potential patient concerns to monitor in order to ensure continuity of medication therapy.

  • Maintains neat and clean working environment
  • Demonstrates knowledge and personal mastery of required job functions, internal communications and operating systems; i.e. Clinical Pharmacology, UP-TO DATE, Meditech, ADM’s.
  • Accountable for maintaining and improving own job knowledge, skills and competence.
  • Completes annual education requirements and competencies/skills assessments as needed. TB test as scheduled.
  • Education profile is completed and updated on an on-going basis on the individual attendance program form.
  • Attends all staff meetings on scheduled work days and if scheduled off, reads and signs-off the minutes of those meetings.
  • Attends all mandatory in-services as scheduled.
  • Participates in department or hospital committees, task forces, and/or provide departmental education.
  • Assists with the orientation of new employee(s) and/or orients current employee(s) to improve in a specific area.
  • Demonstrates knowledge and use of department QI by identifying problems and offering solutions.
  • Investigates inconsistencies in drug therapy including recognition and documentation of possible adverse drug reactions
  • Work with facility Clinical Coordinator to improve documentation of clinical pharmacy interventions
  • Participates and documents information as requested for Medication Use Evaluations or Drug Studies.
  • Coordinates and serves as an information source for medical and hospital staff regarding the effects and use of medication.
  • Promotes and recommends appropriate, cost-effective alternatives to drug therapy.
  • Interfaces with physicians and nurses to develop quality, cost effective therapeutics through clinical pathways.
  • Performs other duties as deemed necessary by the Director in accordance with established policies and accrediting agencies.
  • Fulfills the responsibilities of a staff pharmacist when called upon. (SEE Staff Pharmacists description)

Qualifications:

  • Licensed Tennessee Pharmacist.
  • Post graduate Emergency Medicine residency, equivalent experience, and/or advanced training preferred
  • 3 years experience as a Clinical Pharmacist in a hospital setting
  • 3 years of experience in area of specialty highly recommended

Degree of Supervisor Required:Minimal -individual proceeds on his/her own, in compliance with policies, procedures, and practices prescribed by immediate supervisor.

Employees Supervised: Pharmacy Technicians and other pharmacy staff as needed.
