EMERGENCY DEPARTMENT CASE MANAGER
Join our ER team at Moab Regional Hospital

The role of the Case Manager will be to support patient connection to services and liaison with an interdisciplinary team of providers, including providers, nursing staff, social work and therapists. Assisting with discharge and follow-up needs for patients is a primary responsibility of this position. This individual is located in the in the Emergency Department and works collaboratively with the hospital's Licensed Clinical Social Workers (LCSWs).
This is a non-exempt, full-time, benefitted position.
Anticipated Schedule: Mid-shift hours. Case Management requires 7-day/week coverage between two Case Managers.
Starting Pay Range: $24 - $26/hour, DOE
Reports to: ER Nurse Manager
DUTIES AND RESPONSIBILITIES:
  • Oversee follow-up for his/her patients, with the goal of a warm hand off to follow up services
  • Participation in data gathering initiatives
  • Pro-actively coordinate follow up when needed
  • Work with a team of medical and mental health professionals in all MRH departments, to include Emergency Department, MRHC, Medical-Surgical and OB departments, as well as the Moab Regional Recovery Center.
  • Coordinate doctor’s appointments and other care (including external case management programs) that aligns with patient care need. Collaborate with community organizations for further resource identification.
  • Main point of contact for MRH shuttle van scheduling
  • Compliant with Healthcare regulations, to include privacy under HIPAA / 42CFR Part 2.
  • Follow-up inquiries with patients and caregivers to assure post-discharge needs are addressed
  • Encourages a trauma-informed approach, as appropriate, to patient interactions
  • Other duties as assigned
EDUCATION AND EXPERIENCE:
  • High school diploma or GED
  • Experience in healthcare, case management, social work preferred or other associated field
  • Computer skills for managing data input, using medical record, and scheduling appointments and MRH shuttle van reservations
  • Ability to relate in a supportive and non-judgmental manner with persons in distress
  • Bilingual in English and Spanish, or English and Diné, is extremely valuable
OTHER REQUIREMENTS:
  • Ability to function in a fast-paced and stressful environment, as needed
  • Organized and resourceful self-starter; strong ability to work in a team.
  • Interest in and motivated to work closely with community resources on patient behalf.
  • Strong interpersonal skills and communication abilities due to nature of liaison work on interdisciplinary team.
  • Compassion for and interest in working with vulnerable populations, including patients with mental health issues and addiction, those with limited English language, and specific cultural needs.
  • Dedication to maintaining strict confidentiality (ethical and regulatory requirement)
  • Non-judgmental attitude and firm personal boundaries.

PHYSICAL REQUIREMENTS:
  • Sitting for long periods may be necessary while using the computer and talking on the phone.
  • Alternately, there may also be busy times when CM is required to meet with patients and various community organizations and hospital staff.
  • Ability to lift up to 25 lbs.
  • Current influenza vaccine or mask-wearing is required. Compliance with all Infection Control policies
