Ammonoosuc Community Health Services was established in 1975 by Dr. Jennifer Lewis, a visionary who recognized the need for health care services in rural norther New Hampshire. Over the years since inception, ACHS found a permanent home in Littleton, NH (1994), expanded its reach by adding care delivery sites in Warren (1994), Whitefield (1998), Franconia (2002), and Woodsville (2007). We pride ourselves in being able to provide integrated care in both the Medical and Behavioral Health realms, allowing our patients to get all of their care under one roof.

ABOUT THE POSITION:

The Early Childhood Development - Community Health Worker will provide comprehensive support services to healthcare providers, children with developmental needs, their families, and other community-based organizations.


The position includes both healthcare and community settings. The work will involve direct contact with people requiring empathy and sensitivity. The ECD – CHW will be responsible for establishing and maintaining interpersonal relationships with health and human service providers, children with developmental needs and families. The ECD – CHW is a frontline public Health Worker who has a close understanding of the community they serve. This trusting relationship enables them to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery for child with developmental challenges and their families. This individual will function in a multidisciplinary team both internally and externally advocating for children with development challenges and their families.

ESSENTIAL RESPONSIBILITIES


Related to Early Childhood Development

  • Promote the mission, vision, and values of the organization.
  • Enhance Care Integration, Coordination, Community Outreach, Education as it relates to the
  • Engage caregivers and/or families in recommended developmental screening and follow-up services.
  • Improve access to recommended developmental evaluations and appropriate follow-up services for children 0-5 when a developmental screening identifies an area of concern.
  • Enhance or expand strategies to assess and address caregiver health and health-related social needs, including incorporating the two-generational approach into your current model of care.
  • Support the development of approaches (e.g., caregiver outreach and education, team-based care coordination, and workforce development) that advance equity in access to recommended developmental screening and follow-up services.
  • Leverage health IT, including electronic health record (EHR) systems, to improve equitable access to recommended developmental screening and needed follow-up services.
  • Integrate individual and Population health (e.g., stratifying by risk, identifying data-informed strategies to reduce disparities, assessing health-related social needs).
  • Provide Parenting support and information coordinate with other community-based organizations focused on the same population.
  • Provide Care coordination and engaging Early Childhood and family partners.
  • Provide Early learning resources to all stakeholders engaged in the child’s developmental needs.
  • Recommend Changes in clinical workflows that enhance access to resources for children, parents, and healthcare professionals.
  • Facilitate Capacity building among health center staff.
  • Enhance the Access recommended developmental screening and needed follow-up services.

Related to general Community Health Worker

  • Work with other members of the team to assess and address the needs of the client and family.
  • Maintain updated client records with plans, notes, appropriate forms, or related information no later than 24 hours after patient contact.
  • Facilitate client access to information, including formal and informal support systems, including web based or print information.
  • Make appropriate referrals to health and social service agencies. Referrals may include application assistance and client advocacy within and external to the organization.
  • Assist clients in obtaining medications through prescription assistance programs and other means.
  • Provide regular client contact via in person visit, by phone, or in writing to provide patient and family support and education and ensure they have completed required or recommended actions.
  • Distribute flyers, brochures, or other informational or educational documents to inform members of a targeted community.
  • Refer community members to needed health and social services.
  • Attend community meetings or health fairs to understand community issues or build relationships with community members.
  • Coordinate classes or otherwise disseminate social, medical, or dental health information to school groups, community groups, or targeted families or individuals, in a manner consistent with cultural norms.
  • Collect data and write brief reports to adhere to program requirements.
  • Participate in training, continuing education, and certification programs.
  • Attend internal team meetings (e.g. All Provider Meeting, CHI Team, Site and Department Team Meetings)
  • Provide feedback to behavioral health service providers regarding improving service accessibility or acceptability.
  • Other duties, hours and ACHS site placement may be assigned for any of these responsibilities.

REQUIRED KNOWLEDGE, SKILLS & ABILITIES

  • Understanding and experience in Early Childhood education / development especially from birth through 6 years of age.
  • Understanding of the communities being served – community connectedness.
  • Ability to accept responsibility and accountability for actions.

    1. Google: docs, sheets, forms, and maps
    2. Zoom
    3. Drupal-based website admin functions
    4. Divi-Builder website admin functions
    5. Trello

EDUCATION AND EXPERIENCE REQUIREMENTS

Associate degree preferred in Early Childhood education / or development). Two or more years of related experience in Early Childhood education / development and / or Community Health environment.


CERTIFICATES & LICENSES REQUIREMENTS

Current driver’s license with clean driving record and insurance verification.

Reliable transportation required.


WORK ENVIRONMENT

Frequent travel in state is required. Some evening and weekend hours may be expected. Physical activity demands include regular requirements for standing, walking, lifting objects up to 25 pounds, and ability to use hands to control objects such as keyboards. The position does not normally involve exposure to biohazards or other materials for which universal safety precautions apply.

While performing the duties of this job, the employee is regularly required to see, talk, hear and speak. A great deal of the professional day is spent walking, standing, moving, and manipulating equipment, as well as lifting and moving objects, equipment, and supplies. The employee frequently is required to use hands to finger, handle or feel, reach with hands and arms, and repetitive keyboard motion. In the performance of certain patient care tasks, the employee may be expected to have sufficient manual dexterity to safely perform various skills and techniques. The employee is required to sit and stoop, kneel, or crouch. Specific vision abilities required include close vision, distance vision, ability to adjust focus, and peripheral vision. It is anticipated lifting 20-25 pounds in any given day. Lifting up to 50 pounds may occasionally be required, depending on the type of care provided.
