I) Job Purpose
Under the direction of the BH Forensic Services Division, this position provides clinic support to client in the JAIL, Enhance MH Services Docket, the FACT Team, and other court programs, The position of identify and divert participant at the pre-trial stage of the court process. Diversion Navigator will assist in the referral, screening, and support to individuals navigating through the court process. This position will serve as the primary point of contact for referrals for treatment as provided through the Diversion Initiative FACT and EMHS Docket programs.
II) Essential Functions
Will process and review all inmates on the misdemeanor admissions list.
Performance Requirement(s):
- Review all inmates on the misdemeanor admissions list, i.e. the arrest record, other criminal justice information, as well as mental health history upon receipt 100% of the time.
- Check the CMHC system for data concerning the inmate 100% of the time.
- Inform court staff of referrals to the appropriate court program(s) immediately to ensure that the cases are transferred to the court.
- Obtain consents for release of information from each inmate referred or document the client's refusal to sign them 100% of the time.
- Contact prior psychiatric service providers, family members/significant others, and/or other agencies to obtain information on client's psychiatric history, social functioning, and treatment needs.
- Inform MHMR staff and court staff of clients participating in program services.
- Provide feedback to the psychiatrist, supervisor(s), jail staff, and other MHMRTC staff and or medical staff as needed.
Employee primarily assesses inmates on the misdemeanor admissions list for possible admission into court program.
Performance Requirement(s)
- Meet with new referrals and complete a Summary Sheet on each client.
- Will conduct the requisite assessment and provide recommendations to the court.
- Request protocol medications and doctor's appointments for clients who are in the jail system.
- Review all doctor's lists daily and provide information to all doctors regarding the client..
- Review the medication status of clients in the court system on a daily basis and notify supervisor of any issues.
- Provide face-to-face follow-ups and visits with clients in the jail system.
- Sign out clients at release who plead guilty and are sentenced to time served, and provide them with medications, if prescribed, as well as resources and contact information.
Employee reports and documents client services accurately according to the unit and agency procedures, and maintains working log of activity.
Performance Requirement(s):
- All contacts with clients must be documented using an approved progress note format.
- All notes for services are written no later than 24 hours following the encounter with the client.
- Summary sheets are completed and turned in within 24 hours following the encounter with the client.
- TRAS assessments are completed and turned in within 24 hours following the encounter with the client.
- Places all paperwork related to clients in the appropriate areas for inclusion in the client medical record.
Employee meets the required deadlines for completion of paperwork related to non-client aspect of job.
Performance Requirement(s):
- Turn in all SALs from the day prior by the following day. Progress notes much be attached to all SALs.
- Turn in all mileage and timesheets by the required deadline to the Payroll Specialist and/or assigned payroll person.
- Ensure that all credentialing requirements are being met to ensure that the employee maintains credentialing privileges.
- Ensure that all training is up-to-date to ensure that the employee is adhering to agency policy.
Perform other related job duties or responsibilities as requested or required, whether or not specifically mentioned in the job description.
- Exhibits a willingness to assume additional duties; seeks the guidance of supervisor prior to beginning an unfamiliar assignment.

V) Travel
This position requires driving to and from locations for work and/and may require overnight traveling. Employees are required to have a license in good standing and be able to be covered by agency liability insurance. If personal vehicle is utilized for work travel, then maintaining personal auto insurance coverage is required.
