The Montana Department of Public Health and Human Services (DPHHS) has a career opportunity within the Senior & Long Term Care Division, Community Services Bureau.
This position is part of Montana's Money Follows the Person Capacity Building Project and is based in the Community Services Bureau. This role is responsible for the development, oversight, and management of the Diversion and Transition Program (DTP). DTP is intended to implement upstream interventions to ensure the diversion of Medicaid recipients from institutional settings. When institutionalization cannot be avoided, the DTP Manager will work to identify and remediate barriers to transition that cannot be addressed through existing services or programs. This position reports to the Money Follows the Person Project Director.

Qualifications for this Career Opportunity
- Ability to proficiently utilize Microsoft Office Products to include Word, Excel, PowerPoint.
- Understanding of the wider objectives of the program as identified in the project proposal.
- Broad knowledge of program and program management methodology.
- Able to resolve differences of opinion, using tact and diplomacy. Excellent customer service skills.
- Meet minimum qualifications:
- Bachelor's Degree in human services, business/public/health administration, or related fields.
- Two years of professional experience in health care, public health, behavioral health, or general program management.
- Experience with Word, Excel, PowerPoint
Preferred
- Database applications is preferred.
- Experience with Medicaid is preferred.
- Experience with the Montana Medicaid Information System (MMIS) is preferred.
- Experience with Query Path is preferred.
- Other equivalent combinations of education and experience will be evaluated on an individual basis.

This position may be eligible for partial telework within the state of Montana. Specific conditions will be provided by the supervisor and must adhere to state policy.
This position may be eligible for a statutory $1.50 an hour base pay increase effective July 1, 2024.
The primary location for this position is open for consideration contingent on available workspace. Specific conditions will be provided by the supervisor and must adhere to state and DPHHS policies.
This position is grant funded through August 2025.
