Job Summary:
The Discharge Planner's primary responsibility is to screen patients for discharge needs and to ensure a timely and appropriate discharge plan from the hospital is implemented. Works in conjunction with the interdisciplinary team to ensure that patients and their families understand the available discharge options as well as resources that are available to them at the time of discharge. The Discharge Planner will make referrals to outside agencies as well as follow up with those agencies to ensure the patient is appropriate.
Job Functions:
Provides discharge planning to patients and families, helping to determine which hospital or community resources should be included in the patient's discharge plan.
Accurately assess patient's needs: clinical, financial, and psychosocial. Make referrals to appropriate agencies and community resources.
Maintains relationship with physicians and mid-level providers and assists them with discharge plans to ensure a smooth transition of care.
Provide complete and appropriate documentation of the discharge plan as completed by the social worker, patient, and family in Midas.
Co-ordinates with the interdisciplinary team to ensure the needs of the patient as well as the discharge plan is progressing.
Assists with collection of data as determined by the Director and Manager of Care Management (i.e. Variance Days and Sign Date and Time of orders).
Maintain compliance with all Lourdes hospital and Lourdes Care Management Policies and Procedures
Demonstrates own professional development and leadership skills to ensure quality patient care in accordance with accepted professional standards.
Education:
BSW or Bachelors in a related health field
BLS certification required. BSW license in the
state of Kentucky preferred; membership and
participation in a professional nursing
organization encouraged.
