As part of our success and innovation in the field we are seeking qualified applicants for a Discharge Planner/Coordinator (BSW) position.

The Full-TimeDischarge Planner/Coordinator (BSW) provides discharge planning support to the psychiatric case-management department and supports psycho-educational group programming.
Key Job Duties and Responsibilities:
- Initiates discharge planning and arranges aftercare appointments/referrals.
- Completes all needed discharge paperwork with patients and supports at time of discharge.
- May conduct psycho-educational groups and other groups as assigned.

Qualifications
- Bachelor’s Degree in Social Work licensed by the State of Michigan required.
- Strong verbal and written communication skills
- Previous administrative experience preferred - revelvant internship experience will be considered as equivalent
