- Accommodates the ED's busiest time, most likely 11:00-19:30, by flexibility in hours
- Advocates for ethical and holistic care to promote the autonomy, dignity, rights, values and beliefs of those we serve
- Aligns practice with safety and quality. Identifies and corrects problems.
- Arranges transportation for all patients who are discharged to a facility for lower level of care
- Identifies and resolves delays and obstacles in collaboration with the RN Case Managers, nursing and the attending physicians
- Initiates referrals for home health care, hospice, and medical equipment and supplies.
- Makes appointments during business hours for patients to the appropriate clinics, if needed
- Manages all aspects of discharge planning for assigned patients as follows: Meets directly with patient/family to assess needs and develop an individualized continuing care plan in collaboration with the physician.
- Meets with families and patients in the ED who need Discharge planning (DCP)
- Provides a discharge summary to a patient's insurance provider to secure coverage for care
- Refers patients and families to all post-acute services necessary for a safe discharge and follow up
- Remains available to patient after discharge to answer questions or provide new referrals if necessary
- Reviews and discusses the plan with the patient, family, friends and professional staff and ensures understanding and agreement
- Sets up Home Health Care, Hospice, SNF, or any other out patient service needed for the patient, EXCLUDING transfer to another acute care hospital
- Works with the ED Physician to assess the discharge planning needs of the patient
- Works with the ED physician and staff to discharge patients safely
- Utilizes Ensocare to refer to outside services
