Position Purpose:

The Pharmacist will assist with medication reconciliation prior to discharge, offer to obtain and fill the patient’s prescriptions, and dispense the medications bedside with consultation. The Discharge Pharmacist would also be in charge of follow up after discharge to answer questions, check for compliance, and remind patient to get refills if applicable. Adheres to MercyOne’s performance expectations and performs all defined services and other related duties in accordance with the mission, vision and values of MercyOne.

What you will Do:

Medication reconciliation at time of discharge.

Consultation at patient's bedside of new medications.

Patient follow-up phone calls regarding questions/reminder to refill.

Communication with Physicians and documentation.

Introduction of self and Program to new admissions.

Aid Outpatient Pharmacy in the filling process/retrieve prescriptions from Outpatient Pharmacy.

Register patient's into QS1 computer program.

Adheres to all safety, infection control and colleague health policies and procedures.

Based on principles of growth and development, provides care appropriate to the age related needs of the patients served on his/her assigned unit which may include:

Birth – 28 days Neonate

1 – 18 Months Infant

18 Months – 3 Years Toddler

3 – 6 Years Preschool-aged

6 – 12 Years School-aged

12 – 18 Years Adolescents

18 – 65 Years Adults

65+ Years Geriatric

Hours/Schedule:

Monday through Friday

9 am to 5:30 pm

Minimum Qualifications:

Current Iowa Pharmacist license. Mandatory Reporter.

1.Experience is required to include outpatient pharmacy services in a progressively more responsible position.

2. Demonstrated ability to manage, teach, make independent decisions and assume responsibility.

3. Strong interpersonal communication skills, verbal and written, with the ability to communicate and work effectively with all levels of management, physicians, staff, customers and vendors.

4. Working knowledge of computer systems. Need to have experience with reporting, statistical analysis, Excel spreadsheets, Word, and PowerPoint. Need to have a comfort level giving presentations to medical staff and management.

5. Knowledge of JCAHO standards for hospital, and state and federal laws related to medications.

6. Provides high level clinical pharmacy services in a positive environment while working in collaboration with other members of the health care team to maximize the effectiveness of patient care, improve patient outcomes, improve patient safety, and reduce hospital readmission rates.

7. Optimize medication therapy through reconciliation, patient education, and follow-up contact.

8. Futuristic approach to development of programs/systems that support continuous quality improvement and the Wheaton
