Summary
Advanced Healthcare of Hanover is a 46-bed short term skilled nursing facility and looking for a Discharge Nurse Manager to join the team. This Nurse Manager position works closely patients while planning their successful discharge. Through adherence of current federal, state, and local standards, and our commitment to clinical and rehabilitative excellence, you will ensure the highest degree of patient care.
Responsibilities and Duties
- Coordinate all aspects of a patient's discharge/transition.
- Assist with admissions.
- Ensure the smooth transition of patients upon admission and discharge.
- Participate in surveys and inspections.
- Coach, teach and mentor.
- Support AHC culture by promoting “Friendly, Positive, and Focused on Excellence”.
- All other duties as assigned.

Job Qualifications:
- Must be a state licensed PA nurse in good standing.
- Must be knowledgeable of general and rehabilitative nursing and medical practices, procedures, laws, guidelines, and regulations.
- Preferred previous experience in a hospital or health care facility.
- A positive attitude, and a genuine love and care for the geriatric population. Case Manager
Duties:
- Conduct comprehensive assessments of patients' needs and develop individualized care plans
- Collaborate with healthcare professionals to coordinate and facilitate patient care
- Monitor and evaluate patient progress, adjusting care plans as necessary
- Provide education and support to patients and their families
- Advocate for patients' rights and ensure access to appropriate resources and services
- Maintain accurate and up-to-date documentation of patient interactions and care plans
Experience:
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Proficient in computer applications and electronic medical records

Pay: $32.00 - $42.00 per hour
