POSITION SUMMARY: Responsible for Completing FMLA and disability forms for patients in a timely manner.
ESSENTIAL FUNCTIONS:
- Complete disability and FMLA forms based on chart information.
- Transmit forms to insurance companies, employers, patients, etc.
- Interact with patients via phone, patient portal, or email.
- Answer questions for staff regarding forms.
- Request needed information or clarification from providers or their assistants.
- Other duties as assigned
- This is a daytime shift position with the opportunity of remote work.
EDUCATION & EXPERIENCE
- High school diploma or GED
- Minimum of one year experience in customer service.
