SUMMARY:
The Social Worker acts as a resource person to other program staff and to the facility in the area of counseling, resident dynamics/interactions. The Staff Social Worker is under the direct supervision of the Administrator.
DUTIES PERFORMED:
- Conducts psychosocial history to determine issues which need to be addressed.
- Establishes and carries out individualized treatment plan upon completion of psychosocial history if applicable.
- Develops and implements support systems as needed.
- Participates in team meetings and family conferences.
- Maintains records and prepare reports according to facility procedures.
- Coordinates resident's schedule with physical therapy, occupational therapy, therapeutic recreation, nursing service, speech and language pathology, behavioral specialist, dental, vision and other disciplines as needed.
- Assists in patient discharge planning.
- Conducts post discharge follow-up as needed.
- Completes MDS assessments.
- Is an integral part of the interdisciplinary team.
- Any other tasks as assigned by the immediate supervisor.
Job Type: Full-time
Pay: From $45,000.00 per year
