Director of Quality and Population Health

CLASSIFICATION/STATUS: Exempt, Full Time, 40 hours, Permanent

IMMEDIATE SUPERVISOR: Chief Clinical Officer

SUPERVISORY RESPONSIBILITIES: Population Health Department

PAY RANGE: Band 8 ($96,700 - $137,000/year)

The Director of Quality and Population Health will provide oversight and assistance in achieving annual quality goals. The position will also support other functions, such as the regulatory and quality requirements of C3, BILH, and other ACOs. The scope of these functions includes, but is not limited to, Quality, Practice Transformation, Value-Based Care, and EHR optimization. This role must exhibit strong leadership skills, organization and communication skills, independent thinking, flexibility, and a strong attention to detail. Given the Health Center’s size, this role is strategic, and hands-on to directly run reports to have the data needed to drive quality improvement efforts. The Director of Quality and Population Health will supervise the Population Health Manager, Case Manager, and Prevention and Wellness Staff.

YOUR RESPONSBILITIES

  • Support and advise Health Practice leadership, providers, and staff on their transformation efforts towards value-based care.
  • Provide leadership in overseeing improvements in data analytics processes to develop timely, actionable data. This encompasses both data for quality improvement as well as Population Health activities.
  • Manage the data development life cycle and be the primary person running Quality and Population Health reports out of DRVS and EHR systems, discussing results with clinical leadership and subject matter experts.
  • Utilize data analytics and insights to measure the impact of care transformation initiatives that support the health center’s success in team-based care.
  • Monitor and analyze Health Center performance on quality/ACO metrics on an ongoing basis and ensure appropriate communication with providers and clinical leadership. Ensure that any deficiencies are promptly addressed.
  • Support the Health Center/Practice Care teams as practice coaches in locally integrating the Model of Integrated Care and successfully achieving the goals of the Model of Care.
  • Regularly collaborate with each provider and Provider Champions in monthly meeting planning and management.
  • Facilitate communication and serve as the primary point of contact between the organization and ACOs the Health Center participates in.
  • Evaluate, design, and implement Population Health programs. Develop effective tools to measure performance, run reports to analyze related data, and make recommendations to clinical and/or senior leadership based on findings and towards improving the quality performance at the health center.
  • Create and ensure a culture of Quality and safety for the organization.
  • Drive Transformation in Value-Based Care.
  • Partner with Health Center/Practice leadership in continue to transform and improve primary care as the Health Center participates in more advanced value-based payment contracts and models.
  • Support and oversee progress with the Health Centers/Practices in achieving total cost of care and quality performance goals.
  • Understand risk coding, risk stratification, quality improvement, and Population Health and drive improvement in related engagement, outcomes, staff productivity, and financial measures.
  • Work directly with Health Center/Practice staff on achievement of annual key performance indicators through training.
  • Run and analyze performance reports and data to inform decision-making, process, and program improvement.
  • Identify process improvement gaps and work with staff to recommend and facilitate changes.
  • Monitor and identify best practices within the Health Center/Practice; work in conjunction with others to implement and share organization-wide best practices.
  • Work with Health Center/Practice staff to test and refine workflows that support sustainable transformation, and guide practice as they expand interventions to additional patient populations.
  • Provide direct education and training on the Population Health platform, as appropriate.
  • Review regularly Health Center/Practice performance on KPIs in the Population Health platform and other reporting structures, and inform Health Center/Practice leadership on insights, data trends, and leading or lagging performance.
  • Evaluate, design, and implement Population Health programs, develop effective tools to measure performance, analyze related data, prepare reports, and make recommendations to senior leadership based on findings.
  • Collaborate with operations leadership to ensure patient outreach is conducted as needed to bring patients in for needed care to improve performance on quality metrics.
  • Perform other duties as assigned by the Chief Clinical Officer, or designee.

YOUR QUALIFICATIONS, COMPETENCIES, TRAITS

  • Masters in Nursing, Business Administration/Healthcare Administration/ Public Health.
  • Five+ years’ experience in practice re-design work including Patient Centered Medical Home(PCMH), Practice Transformation, Quality Improvement, ACO Development, Ambulatory Care, and Quality and Efficiency Metrics.
  • Three+ years of knowledge and understanding of Value Based Care programs and Risk Adjustment concepts and models.
  • Experience working with Azara DRVS highly preferred. Experience running data reports directly hands on required.
  • Five+ years of leadership experience in practice or project management.
  • Must understand key revenue levers and cost drivers of health center processes.
  • Working knowledge of clinical quality metrics such as HEDIS, NCQA or National Quality Forum.
  • Training in the Model for Improvement, LEAN, or other process improvement methodologies.
  • Experience working on or with primary care teams.
  • Familiarity with Federally Qualified Health Centers and Private Payer/Commercial contracts.
  • Strong analytical and computer skills, including Microsoft Office (Excel and Power Point).
  • Flexible and adaptable to change and demonstrate the ability to work independently.
  • Demonstrated success working as part of a multi-disciplinary team including communicating and working with Providers, Nurses, and Social Workers.
  • Experience working in Value-Based Care and Alternative Payment Models.
  • Excellent written and presentation skills, with the ability to engage, inspire, and build credibility.
  • Must have experience in diverse cultures, with strong commitment to promoting Diversity, Equity, and Inclusion and reducing inequities.
