Job Description
We are seeking a highly motivated and experienced Registered Nurse Director of Case Management to join our healthcare organization. The Director of Case Management plays a critical role in ensuring the coordination and delivery of comprehensive patient care services. This leadership position requires a skilled and compassionate nurse who can manage a team of Case Managers and drive the development of Case management programs that enhance patient outcomes while optimizing resource utilization.
Key Responsibilities:
1. Leadership and Team Management:
- Provide strong leadership to the case management team, ensuring a collaborative and cohesive work environment.
- Recruit, train, and evaluate case management staff to ensure a high-performing team.
- Set clear goals and expectations for Case Managers, fostering their professional development.
- Foster a culture of continuous improvement and quality assurance within the department.
2. Program Development and Implementation:
- Develop and implement case management programs to optimize patient care and resource allocation.
- Collaborate with other healthcare professionals to establish and maintain standards and best practices.
- Analyze data to identify opportunities for process improvement and program enhancement.
3. Patient Assessment and Care Planning:
- Oversee the assessment of patient needs and the development of comprehensive care plans.
- Ensure that Case Managers effectively advocate for patient needs and collaborate with healthcare teams to achieve positive outcomes.
- Monitor and evaluate care plans for their effectiveness and adjust as needed.
4. Resource Utilization:
- Monitor resource utilization, ensuring cost-effective care while maintaining high-quality standards.
- Oversee the utilization review process to optimize utilization of services.
- Collaborate with finance and administration to manage departmental budgets effectively.
5. Compliance and Regulations:
- Stay current with relevant healthcare regulations, accreditation standards, and case management guidelines.
- Ensure compliance with all applicable laws and regulations, including CMS guidelines.
- Develop and implement policies and procedures that align with regulatory requirements.
6. Interdepartmental Collaboration:
- Foster positive relationships and communication with other departments and healthcare professionals to ensure seamless care transitions.
- Collaborate with physicians, nurses, social workers, and other healthcare professionals to facilitate efficient patient care.
7. Quality Improvement and Reporting:
- Establish performance metrics and key performance indicators (KPIs) to measure the effectiveness of Case management programs.
- Analyze data to identify trends, develop action plans, and report outcomes to senior management.
- Implement strategies to continuously improve the quality of care and patient outcomes.
Qualifications:
- Current and valid Registered Nurse (RN) license.
- Bachelor of Science in Nursing (BSN) required; Master of Science in Nursing (MSN) preferred.
- Certification in Case Management (CCM or ACM) strongly preferred.
- A minimum of 5 years of experience in healthcare, with at least 2 years in a case management leadership role.
- Proficiency in healthcare informatics and electronic health records (EHR) systems.
- Strong leadership, communication, and interpersonal skills.
- Excellent problem-solving and critical thinking abilities.
