```Responsibilities```
- Assist individuals with daily activities such as personal hygiene, meal preparation, and medication administration
- Provide companionship and emotional support to individuals
- Implement individualized care plans and ensure the well-being and safety of each individual
- Assist with transportation and accompany individuals to appointments or community outings
- Maintain accurate documentation of services provided and any changes in individuals' conditions
- Collaborate with a team of healthcare professionals to ensure coordinated care
```Qualifications```
- Previous experience in healthcare or Direct Support is preferred but not required
- Ability to follow instructions and work independently
- Strong communication skills and the ability to build rapport with individuals and their families
- Knowledge of basic medical procedures such as first aid and CPR is a plus
- Ability to lift and transfer individuals who may have limited mobility
- Valid driver's license with a clean driving record
