Now hiring a full time Direct Support Coordinator to support adults with intellectual and developmental disabilities in in the Tracy area for our Independent Living Services Program. We are looking for individuals who live in the Tracy (or surrounding) area. We are looking for caring and compassionate people with a high level of integrity to work in our Independent Living Services program as a Direct Support Coordinator, assisting adults in their homes and communities with maximizing their independent living skills. This is a very flexible position with most all work conducted during the weekdays at client residencies, in the community, virtually, and from home.
ABOUT THE POSITION:
We have an immediate openings for an experienced Direct Support Coordinator supporting people in our Independent Living program. We have both full time work available. This program is rapidly growing and offers opportunities for career advancement.
Services provided are dependent on the needs and preferences of the individuals we support, but may include assistance with:
- Grocery and household shopping
- Meal preparation and cooking
- Safety awareness and preparedness
- Budgeting and bill payment
- Household maintenance and organization
- Accessing community resources
- Problem solving
- Social skills
- Medical and dental appointments and follow up
- Pharmacy interfacing and medication management
- Self-advocacy
- Transportation
- Learning and exercising personal rights and responsibilities
- Other supports as needed to help individuals successfully meet their goals

REQUIREMENTS:
- Must be 18 years of age or older with a minimum of a high school diploma or its equivalent
- Must have reliable transportation
- Must possess a valid CA drivers license and insurance
- Must pass a drug and background screening
