SUITES AT ROUSE
POSITION PROFILE
TITLE: DIRECT CARE WORKER (Including and replacing Memory Care Companion)
DEPARTMENT: NURSING - Non-Secure Unit and Memory Care Community

OBJECTIVES:
To assist residents in the non-secure and secure portion of The Suites at Rouse in a personal care environment with various daily functions and provide routine care, which may include transfers, assistance with walking, personal hygiene, nutrition and assistance with meals, passing medications, encourage participation with activities and other daily tasks in a geriatric environment. Direct Care Workers will ensure that the physical, medical, psychiatric, spiritual, emotional, recreational, and social needs of the residents are met and maintained on an individual basis. An outline of essential duties of the position is listed below.
RESPONSIBILITIES:
1. Provide hands on assistance to residents, such as bathing, showering, dressing,
grooming (includes nail care, oral hygiene, personal hygiene), ambulation, transferring, monitoring and/or passing of medications, bed making, minor treatments, incontinency care, as directed by charge personnel.
2. Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit.
3. Responsible to do and oversee assigned cleaning duties to common areas.
4. Make independent decisions regarding resident needs.
5. May assist residents in daily living activities, distributing newspapers, writing letters, and placing phone calls.
6. Chart notes in an informative and descriptive manner that reflects the care provided as well as the resident's response to the care.
7. Will accept and perform duties as assigned on task list.
8. Forward new diet orders, and/or diet changes to the dietary department.
9. Responsible for documenting care offered and provided, as well as, incidental daily occurrences, such as resident outings, visitations, etc.
10. Responsible for notification to pharmacies for refills on prescriptions or notification to physician for expired orders.
11. Prepare necessary lab sheets for resident blood draws.
12. Verify the identification of the resident before administering medication, treatment, etc.
13. Assist residents with menu selections.
14. Assure that resident is escorted to the entrance for transport.
15. Set an example of Rouse Estate Care Values and encourage shift staff to be need accountable to same.
16. Responsible for the collecting, washing, drying, folding, and returning of residents personal laundry, as directed by supervisor.
17. Checking and recording vitals.
18. Administer professional service such as: applying and changing dressings/bandages, packs, colostomy and drainage bags, sputum, catherization, urine specimens, care of the dead/dying, etc., in accordance with established policies and procedures.
19. Communicating and change in resident's functioning and call for 911 assist to hospital if necessary.
20. Follow established infection control and aseptic techniques at all times and when isolation procedures become necessary.
21. Assume the authority and accountability of directing resident assistant on all shifts, including assigning specific duties for resident care and staff.
22. Responsible to maintain medication keys on is/her person during assigned shift and to transfer keys at beginning and end of shift.
23. Responsible to act as the facility charge person as scheduled.
24. Responsible to unlock and lock all exit doors, as well as checking alarms on all exit doors throughout the shift.
25. Assist with the set-up, leadership and tear down of activity programs.
26. May be asked to complete other duties as assigned.
27. Required to complete and pass the medication administration program and orientation.
28. Monitoring and passing medications after medication orientation/completion.
29. Monitor medication cart and resident medical files as per regulation requirements.
INITIATIVE:
1. Report on and off duty.
2. Create and maintain and atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the facility.
3. Check resident charts for specific treatment, medication orders, schedules, etc., daily.
4. Perform routine charting as required.
PRODUCTIVITY:
1. Make written and oral reports/recommendations to the Administrator concerning resident care for shift.
2. Assist in standardizing the methods in which work will be accomplished.
KNOWLEDGE OF WORK:
1. Interpret department policies and procedures to personnel, residents, visitors, family members etc., as necessary.
2. Ensure that resident's rights to fair and equitable treatment, self-determination, individuality privacy, property and civil rights, including the right to wage a complaint, are followed by all department personnel.
3. Maintain CONFIDENTIALITY of all pertinent resident care information to assure resident rights are protected.
4. In cases of death, follow procedure outlined in our established policy.
5. Evaluate emergencies and determine emergency measured needed.
6. Fill out and complete accident/incident report forms on all resident accident/incidents and chart such information on the resident's notes as outlined in Policy and Procedure.
7. Attend and participate in supervisory and in-service meetings.
8. Take TPRs, blood pressures, etc. and chart as required.
9. Prepare/administer medications as ordered by the physician. (Note: Medications set up by one nurse MAY NOT be administered by another nurse.)
10. Answer telephone inquires as outlined in our established policies and procedures.
11. Complete required recordkeeping forms/charts ion the resident's chart. File in the resident's chart and/or forward to Administrator.
12. Order prescribed mediations, supplies, and equipments as established by out policies and procedures.
13. Write resident charge slips on census sheet.
14. Maintain the daily census report and submit to the supervisor.
15. Review physician's orders for completeness.
16. Transcribe physician's orders to resident treatment plans, etc., to assure that they remain current.
17. Requisition and arrange for diagnostic and therapeutic services, as ordered by the physician, either within the facility or by outside arrangements, in accordance with established policies and procedures.
18. Assure that narcotic records are current at all times.
19. Notify physician changes or resident's condition.
20. Review medication card for completeness of information, accuracy in the transcription of physician orders, and adherence to stop order policies.
21. Report all discrepancies of physician's orders, diet, charting, etc., to the RN Supervisor or Administrator.
COOPERATION:
1. Communicate regularly with first and second shift charge staff to keep accurate understudy of any changes or problems solving needs that affect all shifts.
2. Gives/receives nursing shift report upon reporting in and ending shift duty hours.
3. Assist in orientation of newly hired employees as well as supervise and instruct other personnel.
4. Assist others as needed to maintain quality resident care.
5. Assist others in heavy lifting. Use proper equipment.
6. Ensure that resident's rights to fair and equitable treatment, self determination, individuality, privacy, property and civil rights, including the right to wage a complaint are followed at all times.
SAFETY/HOUSEKEEPING:
1. Assure that department personnel, residents, visitors, etc., follow established policies and procedures to include safety rules and regulations, at all times.
2. Assure that stock levels of medication, medical supplies, equipment, etc., are maintained at adequate levels at all times. Requisition equipment and supply needs as required.
3. Evaluate emergencies and determine emergency measures needed.
4. Assure that the facility is maintained n a clean and safe manner for resident comfort and convenience by assuring that necessary equipment and supplies are maintained to perform such duties/services.
5. May perform minor housekeeping duties around the nursing unit.
ATTENDANCE/PUNCTUALITY:
1. Report on and off duty at break times and meal times.
2. Report for duty on all scheduled days.
3. Must arrive on time.
WORKING CONDITIONS:
1. Works in well-lighted/ventilated work areas, as well as throughout the nursing service areas.
2. Sitting, standing, bending, lifting and moving intermittently during working hours.
3. Subject to frequent interruptions.
4. Involved with residents, family members, personnel, visitors, etc. under all conditions/circumstances.
5. Subject to hostile and emotionally upset residents, family members, etc.
6. Communicates with nursing personnel, and other department supervisors.
7. Willingness to work beyond normal working hours, and in other positions temporarily, when necessary.
8. Attend and participate in continuing educational programs as directed.
9. Subject to falls, burns from equipment, infectious diseases, substances, odors, etc., throughout the work day.
10. Be economical and careful with supplies, equipment, etc.
11. Follow all of our established policies and procedures, to include nursing care procedures, safety regulations, personnel policies, etc., to assure that quality resident care can be maintained.
QUALIFICATIONS:
1. Education:
Must possess, as a minimum, a high school diploma or GED and be 21 years of age.
2. Experience: None required. On-the-job training provided. Must be able to pass Department of Public Welfare Medication Administration Training and Testing and complete a minimum of 6 hours of Dementia training annually.
3. Specific requirements:
A. Must possess a desire to serve the ill, aged, and infirm.
B. Must be able to read, write and speak the English language.
C. Possess the ability to make independent decisions when circumstance warrants such action.
D. Possess the ability to deal tactfully with personnel, residents, family members, visitors, and government.
E. Maintaining the care and use of supplies, equipment, etc., of the nursing service department.
F. Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level they are currently functioning.
G. Must be willing to accomplish tasks in a manner that is consistent with the mission, philosophy and personnel policies outlined by the management of the Suites of Rouse.
H. Must adhere to dress code requirements.
I. Must be comfortable with geriatrics and comfortable in assisting them with personal care needs.
PHYSICAL REQUIREMENTS:
A. Must be able to move intermittently throughout the work day.
B. Must be able to speak the English language in an understandable manner.
C. Must be able to cope with mental and emotional stress of the position.
D. Must possess sight/hearing senses, or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.
E. Must function independently, have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
F. Must be in good general health and demonstrate emotional stability.
G. Must be able to relate to work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.
H. May be required to lift residents, medical equipment, supplies, etc.
