We are looking for Direct Care Specialist for our residential treatment program in Holbrook.
A Community Bridges Inc. (CBI) Direct Care Specialist is responsible for assisting with the admission and orientation of participants of the program. Direct Care Specialist is also responsible for the ongoing monitoring of each patient in the facility as well as discharges.
Residential
- Completes and documents full admission process for each patient who present for services
- Ensures all participants receive orientation (patient rules, tour etc) to the agency at time of admission.
- Identifies, documents, and stores property of each patient prior to their admission into the program.
- Provide both individual and group services (as assigned) to participants regarding activities of daily living and personal care services and any other groups that are assigned
- Facilitates hourly rounds
- Assist all participants with medication self-administration for all prescribed medications in the absence of the RN.
- Completes discharge and associated paperwork while communicating with the Transition Coordinator.
- Notifies Manager on duty at the first sign of medical and psychological risk level.
- Ensures that each patient receives all of their property at the time of discharge and/or transfer.
- Ensures all patients attend all groups and meetings unless otherwise directed by Manager
- All required documentation is completed prior to the end of the shift
- Other duties as assigned by supervisory or administrative designee
Skills and Requirements:
- Must have a minimum of a High School Diploma or General Equivalency Diploma (GED)
- Must have Level One Fingerprint Clearance Card
- State of AZ EMT Certification, Medical Assistant Certification or Certified Nursing Assistant, preferred
- In recovery and have 12 consecutive months of recovery from substance abuse and/or GMH, preferred
- 2+ years behavioral health/substance abuse experience, preferred
- Valid driver's license and clear 39 month MRV record, preferred
