Dialysis Social Worker Lmsw

We are seeking a Dialysis Social Worker to join our team!

Responsibilities:

• Provides psychosocial services to all patients treated by the facility (in-center and home hemodialysis)
• Assess nature of patients' situations by interviewing and reviewing personal history
• Works with the healthcare team to promote positive adjustment rehabilitation and improved quality of life for our patients
• Develops and maintains positive relationships through effective and timely communication to all relevant parties
• Facilitate referrals to other healthcare professionals and programs
• Maintain accurate client documentation

Qualifications:

• Previous experience in Dialysis Social work. counseling, or other related fields
• Compassionate and caring demeanor
• Ability to build rapport with clients
• Strong leadership qualities
• Excellent written and verbal communication skills
