Job description

At Summa Rehabilitation Hospital, we are passionate about patient care!  At our 60-bed hospital, we provide inpatient rehabilitative services to our patients who are often recovering from disabilities caused by injuries, illnesses, or chronic medical conditions.

We are seeking a PRN Dialysis RN to join our team of passionate patient caregivers!

Develops, implements, evaluates and revises a plan of care of assigned patients and families while promoting the mission and values of Ernest Health. Reviews all patients receiving dialysis therapy and assesses clinical status throughout treatment. Consistently assists team members in the delivery of acute hemodialysis utilizing the established standards of care. Areas of practice are diverse, including, but not limited to, the fields of hemodialysis and peritoneal dialysis. The provision of care includes direct care along with the delegation and supervision of all nursing care in accordance with the applicable Nurse Practice Act. Specific components of the Registered Nurse role include demonstrating clinical competence; managing patient care; improving quality of care; establishing professional relationships and utilizing professional skills; fulfilling responsibilities of the role of the hospital; and developing clinical expertise per specialty. Integrates the hospital’s mission and “Guiding Principles” into daily practice.

ESSENTIAL FUNCTIONS:

1. Remain current with knowledge in the field of medical/surgical nursing and progressive care nursing. Demonstrate willingness to modify behavior and/or professional practice based upon feedback and professional standards review. Participates in staff and other facility team meetings. Responsible for direct patient care of assigned patients in the inpatient dialysis services setting and for providing the appropriate treatment technique for assigned patients as prescribed in all modality procedures, as appropriately defined-hemodialysis and peritoneal dialysis. Reviews all patients having dialysis therapy. Reviews the physician orders, present weight, laboratory values, and discusses the patient’s present status. Assists as necessary to provide a safe and comfortable experience for the patient while on dialysis. Contact the nephrologist directly as necessary. Performs treatments and administers medications based on physician orders, CMS standards, and established company/hospital standards of care.

2. Develop, document, and implement individualized care plans for assigned patients, utilizing resources of interdisciplinary team and family when appropriate, and considering developmental life stage of patient. Accurately and consistently utilize the existing plan of care. Assesses patient’s response to treatment therapies making appropriate adjustments and modifications to the treatment plan as indicated by the appropriately credentialed physician. Communicates problems or concerns to the inpatient program manager, appropriately credentialed physician, primary nurse or other designated personnel.

3. Apply all elements of effective medication administration including patient identification, medication confirmation, vital signs and/or lab values, time frames, IV rates and sites, and all policy and procedure guidelines. Responsible for the implementation, administration, monitoring and documentation of the patient’s response to prescribed intradialytic transfusions, including appropriate notification of adverse reactions to physician and blood supplier.

Minimum Education and Experience

· Current, valid, and active license to practice as a Registered Nurse in the state of employment required

· Current BLS certification required

· Current ACLS certification required within six (6) months of hire.

· Two (2) years of dialysis experience or one (1) year of Critical Care experience or one year of dialysis experience and two years of experience in the Medical/Surgical field required.

· Ability to maintain proper levels of confidentiality.

· Ability to work closely and professionally with others.

· Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals as appropriate to the position.

· Freedom from illegal use of and effects of use of drugs and alcohol in the workplace.
