The Clinic Nurse Navigator (RN) will collaborate with the interdisciplinary team to grow, develop and support the preventative health measures that support compliance by our patient population. Duties will include triaging and facilitating care to patients, patient and staff education, data collection, protocol development, ensuring compliance with guidelines and current measures by insurance payor. The Clinic Nurse Navigator must be able to work in a self-directed environment, with the willingness and ability to collaborate with other members of the team for both primary care and the specialty clinic.
JOB DUTIES:
- Provides education as needed to the patients/family related to their chronic care diagnosis, diagnostic procedures, medication and other complex aspects of the plan of care when appropriate.
- Is accountable for the quality of nursing practice and functions as the primary contact for all insurance payors and to CRMC Administration.
- Functions independently providing direct and indirect patient care based on the nursing process for the patients in CRMC Medical Group.
- Will collaborate with all providers involved in the patient care treatment plan by utilizing knowledge of patient assessment, planning, intervention, evaluation and insurance payor databases.
- Will provide nursing services as needed for day-to-day patient care within the clinic setting such as implementing physicians’ orders, administering medications, performing treatments, procedures and special tests, and documenting treatment as required by company policy and local/state/federal rules and regulations.
- Assist in having patient complete diagnostic tests ordered or prescribed by provider in timely manner
- Assess and evaluate patients' needs for, and responses to, care rendered
- Apply sound nursing judgment in patient care management decisions
- Collaborate with the clinical team to create a Plan of Care for all patients
- Maintain confidentiality of all information according to HIPAA and company policy

EDUCATION AND EXPERIENCE:
- Graduate of an accredited School of Nursing, required
- Current Registered Nurse licensure with the State of Kansas, required
- BLS, required
- Valid driver’s license, required
- Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
- Familiar with database metrics for quality reporting and tracking
- Bachelors degree, preferred
