Southwest Behavioral & Health Services is seeking a Community Outreach Navigator to join our Community Resilience Program! This position will give you the opportunity to make an immediate and meaningful impact in the lives of our members we serve.
In this position you will be supporting members in Phoenix, Arizona at our Criminal Justice Services community with maintaining and improving their physical and mental health. This position will report to the Program Coordinator who will support you and your team's success. The work schedule will be Monday through Friday with a focus on flexibility and work-life balance.
Ideal Candidate
Are you new to Criminal Justice or have a few years of experience? Maybe you are a new graduate with a Psychology degree and internship experience. If so, we want to speak to you!

The Criminal Justice Navigator is a specialty position working with adult individuals who frequent the legal system/ and or are transitioning within or from jail or prison systems. Position works as part of a multi-disciplinary team with the County Jail System, City, PNO Case Managers, and RBHA to successfully transition persons from jail or prison to services and community stability.

About the Position
The Criminal Justice Navigator is a specialty position working with adult individuals who frequent the legal system/ and or are transitioning within or from jail or prison systems. Position works as part of a multi-disciplinary team with the County Jail System, City, PNO Case Managers, and RBHA to successfully transition persons from jail or prison to services and community stability.
Duties & Responsibilities
- Works to provide and or facilitate the prescribed treatment plan accordingly
- Develops working rapport as to effectively perform all job functions.
- Provides crisis intervention, de-escalation, and Behavioral Health interventions.
- Assesses for and provides Integrated Care based referrals and assistance to obtain healthcare, medications, medically necessary equipment.
- Provides jail visits to meet and prepare persons for court.as needed or assigned
- Reviews the expectations/offers for treatment/ gains agreement for treatment with persons
- Act as an Advocate in court for persons served
- Works in Real Time in accordance with the courts schedule
- Responsible for case management and carries an independent case load.
- Plan, coordinate, and assist client with immediate needs/ basic needs for success
Minimum Requirements
- Must be at least 22 years of age
- Have a high school diploma/GED and two years ofbehavioral health/integrated healthcare relevantwork experience; OR
- An Associate's degree in an unrelated field and at least one year of full-time behavioral health / integrated healthcarerelevantwork experience; OR
- A related Associate's degree and six monthsof behavioral health / integrated healthcare relevantwork experience
- Have and maintain a valid Arizona driver's license and are able to pass a motor vehicle screen for insurance purposed in order to transport clients
- Level I fingerprint clearance issued by the Arizona Department of Public Safety or be eligible to receive within eight (8) weeks of hire (as required per program requirements)
