Essential Job Functions:
- The Nursing Coordinator will perform the essential job functions of a Nurse Case Manager
- Participate in the training of new Nurse Case Managers and per diem nursing staff in the policies and procedures involved in Nurse Case Manager and per diem nurse job functions.
- Orient new Nurse Case Managers in job functions until competency determined and act as a continuing resource for new Nurse Case Managers.
- Manage the staffing schedule of the health center.
- Coordinate the Peer Review Process for record review and MAR review.
- Assure compliance with the Vaccine for Children Program.
- Attend meetings in the absence of the Division Nurse Leader.
- Assure compliance with routine equipment checks.
- Collect supply order information and review for submission for approval by Division Nurse Director.
- Collect documentation requested for regulatory review and investigations.
- Participate in development of quality improvement projects in conjunction with direct supervisor with overall responsibility for implementation and evaluation of the intervention.
- Complete performance evaluations of supervisees.
- Ensure full compliance with internal and external regulatory requirements; obtain health records, birth record, immunization registry and other pertinent medical or mental health information; review ACS triage material and communicate with ACS on as needed basis.
- Actively communicate with community providers scheduling needed appointments and obtaining health related information for the residents.
- Actively participate in obtaining psychotropic and non-routine medical procedures consents when necessary.
- Serve as a liaison between youth and medication prescriber, provide appropriate medication training to youth and family assisting in the assessment of medication side effects and/or the effectiveness of treatment.
- Audit team member charts to ensure that A29I and ACS requirments are met.
- Communicate and coordinate with clinical team, foster and biological family/guardian about all medical conditions, appointments and/or treatments.
- Participate in community visits (e.g. Emergency Room, hospitalizations, community specialty appointments, home visits) on as needed basis.
- Participate in all staff meetings, rounds, case reviews, FTC, etc.
- Promote the incorporation of the tenets of The Sanctuary Model in the workplace and in interactions
Qualifications:
- Bachelor's Degree required, preferably in Nursing; Master's degree in Nursing preferred
- NYS Registered Nurse License required
- Two years of Medical Surgical experience (pediatrics preferred) or acute and sub-acute care; visiting nurse services preferred
- Bilingual (Spanish) is preferred
Salary: $95K - $100K
