Job Details:

• Location: Granite City, IL
• Company: Sunbelt Staffing, on behalf of a Nationwide Home Health Agency
• Position Type: Contract, 13-week term with possibility of extension upon successful completion
• Schedule: Monday-Friday, 8:00 AM - 5:00 PM with occasional weekends and on-call
• Compensation: Competitive salary, mileage reimbursement, medical, dental, vision, and 401k benefits after 90 days
• Sick pay after 6 months of employment

Job Responsibilities:

• Conduct Home Health visits to patients within Granite City, IL, and neighboring communities.
• Conduct thorough assessments of patients' health conditions, taking into account their medical history and current needs.
• Develop customized care plans tailored to each patient's unique requirements, in alignment with physician directives and best practices.
• Deliver expert nursing care, including specialized interventions such as wound care, medication administration, and health education.
• Accurately and promptly document all patient interactions, treatment plans, and progress notes using Oasis and Homecare Homebase software.
• Collaborate closely with interdisciplinary team members, including physicians, therapists, and social workers, to ensure seamless coordination of care.
• Foster open communication with patients' families and caregivers, providing support, education, and guidance as needed to promote continuity of care and optimal patient outcomes.

Job Requirements:

• Active IllinoisRegistered Nurse (RN) License.
• Active American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) certification.
• Minimum of 1 year of experience in Home Health nursing.
• Minimum of 1 year of experience with Oasis and Homecare Homebase software.
• Excellent clinical assessment and critical thinking skills.
• Strong communication and interpersonal abilities.
• Reliable transportation and willingness to travel within assigned territory.
