Responsibilities:
- Conduct comprehensive patient assessments to evaluate their health status, including physical, emotional, and social factors.
- Develop and implement individualized care plans based on assessment findings, physician orders, and patient goals.
- Administer skilled nursing care, including wound care, medication administration, IV therapy, and other nursing procedures.
- Educate patients and their families on disease management, medication regimens, and self-care techniques.
- Monitor and document patient progress, making necessary adjustments to the care plan as needed.
- Collaborate with interdisciplinary healthcare teams, including physicians, therapists, and social workers, to ensure holistic patient care.
- Maintain accurate and timely electronic medical records in compliance with regulatory guidelines.
Requirements:
- Current and unrestricted Registered Nurse (RN) license
- Minimum of 6 months of geriatric Home Health experience
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills to effectively interact with patients and their families.
- Compassion and a patient-centered approach to care.
- Valid driver's license and access to reliable transportation.
