Continuing Care Specialist
Salary: $50,000 per year

- Work with Embark programs nationwide to develop Continuing Care plans for clients and their families.
- Work with external referral sources and partners, including Education Consultants.
- Embrace the core values of empathy, trusting relationships, service, growth, and results.
- Document Continuing Care best practices and coordinate a seamless transition between programs and referral partners.
- A Bachelor’s degree in mental health or related field.
- At least one year experience in the behavioral health industry either in a case manager or outreach/intake capacity.
- A desire to facilitate client growth through a team approach, working with other care providers.
- Ability to develop trusting relationships with families, our program leaders and referral partners.
- High attention to detail.
- Technology savvy, with experience with video technology and telehealth platforms.
- Ability to work with various referral partners including case managers, educational consultants, and mental health practitioners.
- Preferred – Previous experience working with adolescents, young adults and their families in a clinical outreach or marketing capacity.
- Preferred – prior experience in outreach and marketing.
- Preferred – experience with how insurance works and talking to families about their insurance voerage
- Preferred – experience using Salesforce. Knowledgeable on how to navigate EMRs.
- Ability to pass a state background check.
- This is a full-time remote position, and you must have the ability to travel at least 15% of the year.
