Job Summary:
The Nurse Navigator provides coordination to the Congestive Heart Failure Program to include, but not limited to: Manage clinic equipment and supplies, clinic administered medications, patient nutrition supplies, educational material, linens, patient BP monitors and scales, and medication samples. Coordinate Heart Failure Clinic Practice and Quality Committee Meeting. Develop and update clinic procedures and protocols as well as patient education material. Actively assess performance of clinic and clinic practices. Promote program growth and alignment with updates in clinical practice and guidelines. The nurse works collaboratively with the Clinic Manager, Regional Director of Cardiology Operations, physicians from multi-specialties in developing referrals, coordinating clinic and outpatient visits to navigate clinical care accurately and effectively. The Nurse Navigator is responsible for organizing provider team meetings, compiling clinical data for presentation, and following up on team recommendations. The Nurse Navigator monitors and assures that patient care is in compliance with CMS required clinical registries associated with any procedures.
Qualifications:
Graduate of an approved School of Nursing with a current North Carolina RN license or RN multi-state licensure privilege through the Nurse Licensure Compact. BSN Preferred. Minimum 3 years of experience with cardiac critical care preferred. Congestive Heart Failure experience preferred. Basic leadership skills. Excellent interpersonal and communication skills, written and verbal. Ability to problem solve and multitask. Highly organized in compiling clinical data for presentation to provider teams to accurately assess patients. Diplomatic personality with strong drive to grow the program.
