Position Overview: The Community Transitions Liaison Program Assistant (CTLPA) is a key member of the CTLP team. The CTLP team supports individuals who wish to transition from a Nursing Facility into the community. The CTLP team maintains a regular presence on site at the company area Nursing Facilities to assist patients in determining various LTSS and community supports to facilitate community living. The CTLPA works closely with the Community Transitions Program Manager, the Community Transitions Liaison, and the CAE screening nurse, as well as partnering with nursing facility staff to support discharges to the community. As a member of the interdisciplinary Home Care team, the CTLPA actively collaborates with the team to ensure the provision of person-centered services to older adults, people with disabilities, and their caregivers according to the Mission, Vision, and values of our client.

Position Responsibilities: Supports the CTL to operate at the top of their role and skills, by completing administrative tasks. Accompanies the CTL on nursing facility visits, or visits patients independently as needed. Assist in securing necessary documentation and identification needed for housing applications and other public benefits. Assists in completing application and supporting documentation. Conducts clerical duties as needed by the CTLP team, including data entry into EOEA-designated cloud-based data enterprise system. Communicates with Nursing Facility staff regarding patient referrals and assessments. Ensures case documentation meets the standards set forth by the Executive Office of Elder Affairs for the CTL Program.

Qualifications:

  • High School diploma or equivalent with 1 year of related work experience and/or interest in working with nursing facility residents including older adults or persons with disabilities who are transitioning from an institutional to a community setting.
  • Must demonstrate a passion for supporting consumers in achieving their personal goals related to wellness and community living
  • Ability to make proper judgments, good written and verbal skills, demonstrated leadership qualities (for supervisors/mgrs.) and ability to maintain effective interpersonal relationships with staff members, consumers and partners
  • Possess the knowledge and skills necessary to work successfully in the interdisciplinary team and to provide or arrange for services
  • Knowledge of community resources preferred
  • Proficient in the use of computer systems, including but not limited to, Windows based software application including but not limited to Word, Excel, PPT and Outlook
  • Ability to process information accurately
  • Ability to work both independently and as part of a team
  • Professional demeanor and ability to handle confidential materials with discretion
  • Must have reliable transportation and valid driver's license in good standing for community outreach and home visits
  • Bilingual skills preferred

Hours: Full Time - 40 hours (Monday-Friday)
