Form of workCarolina Collaborative Community Care (4C) is seeking a part time, (10 hours/week maximum), Community Outreach Nurse to work on a PRN basis. The Community Outreach Nurse will represent the organization at designated community based events. The individual will also work with patients per contracted programs and participating physicians contracted with 4C. The nurse will collaborate with providers to develop, implement and prioritize quality improvement and care management initiatives targeting the patient population(s) of assigned contracted program(s) and community-based programs. The individual will also identify patients who would benefit from disease management and case management services; educate and engage patients in their own care management; and link patients to appropriate levels of care/appropriate and supportive services; Associate or Bachelor Degree in nursing with a current LPN or RN license required; CCM preferred. Must have a valid driver’s license and an automobile for use during work. Strong organizational and time management skills, and the ability to work independently are required.
