MTM is looking for a Community Health Worker/EMT in Nebraska.
This role is a mix of remote (work from home) and fieldwork, with visits to the patients home as required based on their needs assessment. Candidates can reside anywhere in Nebraska (including rural areas). Start date for this role is Dec 7th.
What Will Your Job Look Like?
The Community Health Worker/EMT (Mobile Integrated Health) is focused on making an impact in the communities we serve, by conducting member outreach, coordinating care and assisting with identifying and referring to local resources with a goal of improving patient outcomes in conjunction with MTM partner health plans.
This role is community facing and will liaise frequently with both patients and health plans to identify and provide the necessary resources to improve the health and well-being of our members, including health needs assessments, appointment coordination, scheduling transport, housing/ food/ behavioral health needs, and follow-up.
Pay: Up to $30 per hour based on experience
What You'll Do:
  • Establish outreach to members and conduct patient needs assessments
  • Establish long term relationships with assigned members and become trusted advisor for their health and wellness needs
  • Facilitate scheduling and completion of home visits
  • Schedule medical referrals, diagnostic tests and treatments following outlined protocols, assure patients are notified, understand instructions and complete all documentation in EMR and/or required by accepting provider/ agency
  • Complete tracking of referrals to ensure receipt of results and reports
  • Ensure patient notification is completed and follows up with patients
  • Ensure medications and supplies needed for patient care received
  • Conduct spot checks on patients in the field
  • Administer member satisfaction survey
  • Help develop program specific patient education
  • Work with the MIH Director to follow through on service issues, troubleshooting problems and concerns, and complaints
  • Assist MIH Director to develop, implement, review and revise policies and procedures to guide care delivery
  • Accountable for achieving monthly, quarterly and annual patient outcome and satisfaction goals, as established by MIH Director
  • Work closely with MIH Director and other team members to develop/ maintain/ grow relationships with local community-based organizations (CBOs) and providers with a goal of increasing services available for MTM members
  • Assist in developing, and adhere to, simple referral process for providers and CBOs to refer members for necessary services
  • Answer incoming calls from assigned members and provide them with information, materials, and end-to-end assistance with a wide range of services to assist with improving individual health and wellness
  • Build positive relationships with the community, including assisting with local networking and marketing activities
  • Gain knowledge of and understand all aspects of the Client and the contract
  • Monitor patient satisfaction beyond statistical data

What You'll need:
  • Minimum High School diploma or GED equivalent
  • Bachelor's Degree or equivalent, job-related experience preferred
  • Minimum 2 years' experience working in the medical field and knowledge of Community Health, case management and/ or care coordination
  • Experience with customer service
  • Minimum of 1 year of patient engagement experience in a hospital, nursing facility, patient care agency or in-home care setting preferred
  • Experience with home health care, case management, emergency medical services, community paramedicine, or mobile integrated health strongly preferred
  • Experience working with Medicaid and/or State programs
  • Experience with large healthcare programs aimed to reduce hospital utilization, cost of care initiatives, improving HEDIS measures and gap closures a plus
  • Bilingual a plus
  • Must possess a valid driver's licenses and insurance
  • Must be authorized to work in the United States and not require work authorization sponsorship by our company for this position now or in the future

  • Must possess a valid driver's license with good driving record and be able to drive within applicable state or locality with reliable transportation
  • Must be willing and able to attend required training and comfortable meeting patients in their homes
  • Must be easily adaptable to ever changing environment and requirements
  • Ability to solve practical problems dealing with a variety of variables
  • Demonstrable personal traits of a high-level commitment, motivation and energy; team orientation; professionalism; trust; personal integrity; and a demonstration of placing others in a place of high value and treating them with dignity and respect
  • Uphold confidentiality and HIPAA compliance at all times and report violations to Privacy Officer
  • Knowledge of Federal, State and local regulations pertaining to care at home or mobile integrated health a plus
  • Knowledge of managed care, quality improvement, and contracting a plus
  • Willing and able to attend and successfully complete any relevant training or certifications as required by MTM and the Client
