Northeastern Vermont Regional Hospital’s (NVRH) Community Connections Department is looking to add a passionate Community Health Worker (CHW) to the team. Community Connections serves as a bridge between the healthcare system and our social service partners. Our team is person-centered and committed to supporting health and wellness improvement through education and advocacy. NVRH is the foremost provider of healthcare services in Vermont’s Northeast Kingdom, committed to enhancing the health and overall well-being of our community. NVRH offers a comprehensive array of services, encompassing primary and preventive care, specialized surgical services, both inpatient and outpatient care, Community Health Improvement and Prevention, and access to 24 hour, physician-staffed emergency services.

Job Summary:

The Community Health Worker connects the gap between healthcare and social services, increasing access to resources through collaboration with long-standing community partners. The CHW interacts directly with clients, conducting a comprehensive intake, gathering information and mapping of natural resources in order to educate and advocate for the right services and supports, while empowering self-management.

What you’ll do day-to-day:

  • Engage one-on-one with clients to conduct a comprehensive intake to understand the current issues and gauge needs
  • Empower individuals to engage in problem-solving and action planning to meet their goals, using a patient-centered, strength-based approach
  • Collaborate within the pre-existing social service and healthcare infrastructure of our community and foster partnerships with community members such as NECKA, Umbrella, NKHS and more.

To be successful in this role, candidates should have:

  • A High School Diploma
  • Experience coordinating care within a social service and/or a healthcare environment
  • Experience working with a patient-centered strength-based approach
