Job Description
Community Health Workers serve as a bridge between clients who have not been actively engaged in their own health care and staff in local clinics to ensure connection to medical care. CHWs conduct outreach to local clinics to find individuals who have been lost to care, and then conduct outreach in the broader community to find these individuals and re-connect them to medical care. CHWs also conduct outreach to underserved communities identified as at-risk for increased levels of HIV infection including - men and women actively engaged in substance use activities, incarcerated, undocumented, uninsured, homeless, or otherwise in denial of their risk of HIV infection.
Responsibilities including linking individuals to rapid HIV testing services and then determining whether the individual should be linked to PrEP (Pre-exposure Prophylaxis) or to medical care. For clients on PrEP, the CHW will request HIV and lab reports every three months. The CHW will provide supportive services including medical care and/or social service appointments.
Administrative duties:
- Accurate completion of all paperwork
- Develop and maintain database of professional contacts
- Establish and maintain client case records using approved Agency forms, ensuring complete confidentiality for all clients.
- Prepare monthly reports in accordance with agency policies and procedures, and grant requirements
- Maintain and update client information in agency and funder databases (E-Compass and CAREWare).
Required Minimum Education: Associate’s Degree Other: None
Required Years of Related Experience: 1-3 years
- Social services/community services
- A minimum of two years experience in the HIV field.
Required Knowledge, Skills, and Abilities:
(Include any required computer skills, certifications, licenses, languages, etc.)
- HIV treatment education
- Community outreach
- Fluency in Spanish is required
- Demonstrated ability to work with people from diverse backgrounds, especially with members of LGBTQ and interests
- Access to a currently registered car and a valid driver's license.
