Community Health Registered Nurse

Neighborhood Health Center (NHC) is GROWING and actively seeking a Community Health Registered Nurse. The Community Health RN will have responsibilities that include, but are not limited to, working with a multidisciplinary team to support our patients by using a trauma-informed, harm reduction and motivational interviewing approach to support patients in developing and reaching health care goals. RN Tuition Reimbursement Potential: NHC’s FQHC status provides a unique opportunity for tuition reimbursement for Registered Nurses, up to 60% repayment of qualified student loans directly related to a nursing degree. This tuition reimbursement program requires application and is not guaranteed. RNs who are granted reimbursement are required to fulfill a two year, full time commitment to maintain eligibility. Additionally, there is potential for an additional 25% repayment for a third year commitment.

Responsibilities:

• Builds trusting relationship with patients to facilitate access to health care services and meet health needs
• Engages patients to participate in health promotion, maintenance and restoration activities
• Works in partnership with our multidisciplinary team (including providers, nursing, CHWs, care coordinators and others) to advocate for patients and ensure health needs are met
• Assists patients and families with accessing, navigating, and connecting with internal providers and services, outside community resources and programs, and other services as needed
• Revises and reprioritizes plan of patient care, as needed; identifies and addresses any barriers effecting a patient’s ability to adhere to plan
• Conducts community outreach, home visits, shelter/street outreach to identify people who are experiencing homelessness and other social support needs and educates about health needs

Requirements:

• Graduated from an accredited two year nursing school
• Must possess a current NYS Professional Registered Nurse license
• Must have current CPR certification
• The ideal candidate has knowledge of case/care management principles and has the ability to work with a diverse group of patients and understands the impact of social determinants on health

• Excellent judgment and discretion

Hourly rate: $30 - $40 an hour (based on a full-time, 40 hour work week). Individual compensation is based on various factors unique to each candidate, including skill set, experience, qualifications, and other position related components
