Job Description

Position Summary:  The role of the Community Habilitation Coordinator is to oversee and monitor the daily operation of the Community Habilitation program to ensure continuity and quality of services. The Community Habilitation Coordinator also supervises the Community Habilitation staff to ensure all regulatory standards are met; to ensure documentation is accurate, complete, and appropriate for billing purposes; prepares billing for submission to the revenue department; completes payroll for the Community Habilitation staff and completes other activities assigned by the Site Program Director of Family Support Waiver as well as Program Director of Family Support Waiver. Additional duties include monitoring the ISS binder to ensure that all new leases are updated, complete ISS checklists and Budgets annually, as well as keep copies of the payment from individuals.  Assist in completing Brooklyn and Queens Family Reimbursement programs. The Family Support Coordinator ensures all regulatory standards are met, ensures documentation is complete and appropriate The Community Habilitation Coordinator must have the ability to read typed and written correspondence and comprehend written material and must have working knowledge of and be able to utilize computer systems (i.e., Outlook, Microsoft Office, Microsoft Excel, etc.).

Essential Accountabilities:

  1. The Community Habilitation Coordinator (CHC) conducts assessments of the functional skills of new individuals that receive services.
  2. Develops, writes, and oversees the implementation of the Community Habilitation Service Plan.
  3. Develops and maintains resource materials in regard to developmental disabilities and treatment services, as per OPWDD guidance.
  4. Prepares e-time and billing in the designated Electronic Health Record (EHR) as well as submitting payroll and billing to the appropriate departments on a timely basis.
  5. Conducts regular internal reviews of each assigned individuals' files to ensure accuracy, compliance and appropriateness of all documents in the records. 
  6. Assigns cases to Community Habilitation Workers and initiates the hiring of new Community Habilitation staff when required as indicated by enrollment numbers.
  7. Participates in the interviewing, hiring and the training process of new staff.
  8. Provides supervision to their assigned Community Habilitation staff to ensure that there are no issues with their cases and that staff documentation is in compliance with OPWDD regulations.
  9. Completes progressive disciplinary actions, as needed, with the Community Habilitation staff, up to and including termination when indicated.
  10. Intervenes on behalf of the individuals when the individual's advocate and/or other providers identifies a concern or reports a complaint.
  11. Functions as a liaison between the Care Managers and the Community Habilitation staff in regards to the quality and appropriateness of Community Habilitation services as well as other individual issues and needs.
  12. Functions as a liaison between the Waiver Department and other WellLife Network departments, such as Day Habilitation services, when necessary.
  13. Conducts staff meetings, including training sessions.  Participates in regular meetings with assigned supervisor.
  14. Must be able to climb and descend interior and exterior stairs.
  15. Required to participate and attend meetings/trainings at various sites that require following different routes of travel to worksite and must have the ability to transport self to various work sites.
  16. On call 7 days a week, after hours and on weekends to assist staff and families.
  17. Always maintain confidentiality.
  18. Work schedules and location are subject to change based on individual/program needs.
  19. Other duties as requested by your supervisor.
Qualifications

A Bachelor's Degree in Psychology, Health, Education and Human Services for QIDP is preferred, including two years of progressive responsibility and experience working with people with Intellectual/Developmental Disabilities. Must have at least two years of experience working in program supervision and at least one year working in the Intellectual/Developmental Disabilities population. Valid NYS driver's license required along with access to a vehicle.
