Job Summary: The Transition/Community Based Support Specialist provides individual psychiatric rehabilitative skills training and crisis assessment/management in the consumer's home or in the community environment. This service is provided through ongoing assessment, preliminary diagnosis, and ongoing treatment of a consumer's presenting Axis I psychiatric and co-occurring disorder under the supervision of the treating psychiatrist and the Director of Operations. The position provides coordination of community resources to assist consumers in accessing medical, social, educational, financial and other appropriate services that help the individual achieve an improved quality of life, while decreasing the need for inpatient hospitalization and arrests. At this advanced level, consumer caseloads are complex and dynamic with increased need for crisis intervention. Staff members in this position are working under the supervision of a clinically licensed professional who provides ongoing evaluation and oversight of all assessments, diagnoses and treatment. At this level, clinical assignments may be of moderate complexity and are handled with considerable independence, and complicated cases receive closer clinical review and supervision.
Services provided may include individual, family and/or group skills training using evidence-based interventions, crisis assessment, and case management.
Work is performed under general supervision with moderate latitude for the use of initiative and independent judgment.
An employee in this position must:
- Possess knowledge of severe and persistent mental illnesses and commonly co- occurring disorders and case management/service coordination principles.
- Effectively communicate their clinical impressions, assess the strengths and functional challenges of those they serve, and collaborate with racially and culturally diverse individuals to achieve their goals.
- Collaborate professionally with an interdisciplinary treatment team and be adaptable to a wide variety of service environments and rapidly evolving clinical circumstances.
- Possess knowledge of the principles of psychiatric crisis intervention and be prepared to interact with emergency medical and psychiatric service providers to ensure the safety of Integral Care staff and clients.
- Practice self-care and seek assistance to manage secondary traumatic stress and counter transferential effects that may result from service provision with vulnerable populations.
- Possess computer skills necessary to document work performed and demonstrate the ability to work efficiently to complete all assessments and documentation in a timely manner.
- This position may have additional duties associated with providing bilingual communication services and requires the completion of a language competency test.
Primary Responsibilities:
- Identify candidates for the pilot and assess their eligibility.
- Coordinate with community providers and Medicaid managed care organizations to access transition assistance benefits, behavioral health care, physical health services, and other community-based services and supports.
- Initiate pre-transition support and coordination prior to relocation from the hospital to help prepare the participant for community living.
- Provide pre-tenancy housing support, such as obtaining documents, working with participants to locate and apply for housing, working with landlords, providing transportation to view housing options, and obtaining a housing voucher, as applicable.
- Provide evidence-based skills training, including Cognitive Adaptation Training (CAT) and other related services or supports to improve independent living.
- Assess, plan, and implement care strategies that are individualized by participant and directed toward the most appropriate, lease restrictive community-based setting.
- Participate in all training activities and learning community with all stakeholders (HHSC, third party evaluators, etc.).
- Ensure the participant's needs and choices are fully represented and supported by the transition team.
- Submit monthly and quarterly reports to HHSC.
Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education:
Required:
- Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field.
Experience:

- Current and valid driver's license and a clean driving record.
Hours: Full-time.
Workhours: 8:00am-5:00pm.
Workdays: Monday-Friday.
Salary: $22.35 - $23.47 an hour based on education plus an additional 10% Crisis Stipend.
