Job Title: Community-Based Clinical Care Coordinator

Department: 911 Diversion

FLSA Classification: Exempt - Full-time

Location: Hybrid – Community/Office

Reports To: Lead Clinical Care Coordinator / Clinical Manager

Position Summary

Provide follow-up behavioral health care coordination services to clients or others such as parents/guardians concerned about an individual. Follow-up eligibility is based on specific program requirements. The Clinical Care Coordinator works with the caller and/or client to monitor symptoms and functioning while coordinating post-crisis care with existing providers or linking the client to new mental health services.

Essential Functions:

· Maintains contact with individuals to coordinate care, help access community resources, continue to plan for client’s safety, and to aid the person served in improving level of functioning post-crisis.

· Ongoing assessment of mental health symptomology, including suicidal thoughts/behaviors or other lethality risk factors.

· Ensures all care coordination, referral dispositions and outcome documentation is completed for all cases.

· Participate in regular team meetings and individual supervision for case consultations.

· Provides training and ongoing support to clinical staff on follow-up care coordination procedures.

· Provides feedback on care coordination systems and works with Clinical Manager and Program Director to enhance follow-up care coordination procedures.

· Provides protocol support, live call assistance, and case consultation to clinical staff.

· Reviews cases as part of continuous quality improvement and to provide supportive feedback to clinical staff.

· Participates in community activities such as system of care committees and resource events to raise awareness of services.

· Will handle inbound call duties as needed, outreach cases that cannot be handled over the phone. This includes outreaching residencies, shelters, homeless encampments, hospitals, and jails.

· Performs other job-related duties as assigned.

Education, Experience and Skills Required:

· Bachelor’s Degree in Social Work, Psychology, Criminal justice, or social service-related field and/or certified peer specialist credential.

· One year of clinical experience preferred.

· Strong service coordination, team building skills, and excellent customer service skills.

· Applied Suicide Intervention Skills Training-Required - can be completed upon hire

· Working knowledge of the St. Louis Area Mental Health Network, the Crisis Intervention Model, and the DSM-V.

· Thorough clinical assessment skills with strong verbal, written, and interpersonal skills

· Ability to learn and adhere to multiple protocols.

· Ability to use computer system and talk on phone while typing.

· Must work well individually and with co-workers as part of a team. Must be flexible with work hours and special requests from leadership.

· A valid driver’s license is required as some driving and/or transportation may be required to take clients to medical appointments, job sites, social activities, and other community resources. (Required)

· Must own a properly insured, reliable vehicle.
