About the Practice
The practice specializes in comprehensive medical care for seniors aged 60 and above. They cater to a wide range of health conditions, from wellness care to managing multiple chronic conditions. Patient-centered care is the practice’s guiding principle, aiming to help patients achieve their health goals through personalized, attentive care.
Position Highlights
Focus on Quality Care: Providers typically see only 7-10 patients per day, allowing ample time for listening, thinking, and bonding with each patient. This approach ensures personalized care that meets the unique needs of each individual.
Comprehensive Services: They offer ambulatory primary care, integrated behavioral health, chronic disease management, end-of-life planning, in-office lab services, medication management, preventive care, telehealth, and vaccinations.
Supportive Environment: Work with an incredible team of 20 dedicated members in a modern, state-of-the-art facility. The office environment is warm and engaging, providing a comfortable experience for patients.
Hours: Monday through Friday, 8:00 am to 4:30 pm, with 24/7 access for patients. Night and weekend calls are shared, ensuring minimal call requirements.
Candidate Requirements
- Board-certified in Family Medicine, Internal Medicine, or Geriatrics.
- Strong commitment to patient-centered care.
- Experience with senior patients and chronic disease management is preferred.
- Excellent communication and interpersonal skills.
