Job description within each role scope of practice
1. Perform under supervision of a registered nurse who has provided written instructions for client care (POC).
2. Assist client in all activities of daily living/hygiene assistance, bathing, grooming, linen changes as outlined in the POC.
3. Prepare nutritious meals within the patient’s diet and assist client with eating when necessary.
4. Assist clients with transfers, ambulation, and exercises under guidelines of the RN or PT when warranted and documented in POC.
5. Perform light housekeeping chores, which facilitates client’s self-care in the home.
6. Assist client in bathroom or use a bedpan/commode and perform incontinence care.
7. Meet the safety needs of the client and use equipment safely and properly.
8. Complete records and carries out all assignments as required.
9. Notify service supervisor or director of any acute or pertinent changes in a client’s condition
10. Communicate availability to work to the coordinator on a weekly basis or as requested.
11. Maintain confidentiality regarding the client’s condition and his/her family, with the exception of the agency.
12. Perform all skills and procedures competently and within the regulations and licensing laws of the state, agency policies, and other applicable federal and state laws.
