12-hour shifts with every other weekend off (6:00am to 6:30pm)

Job Summary:

This position carries out all assigned para-professional nursing care and treatment services in prescribed and efficient manner.
Provides a safe, therapeutic and hygienic environment. Learns, implements and monitors Arizona Management System (AMS) methods. Uses AMS concepts and tools for problem solving and continuous work/process improvements and efficiencies. Incorporates a cultural focus on Veteran/Member-centered care in the performance of job duties.

Job Duties:
  • Assists the licensed nursing staff and Veteran/Member by performing a variety of care tasks.
  • Helps Veterans/ Members with transfers, ambulation, eating, bathing, dressing, toileting, personal hygiene and other ADLs.
  • Turns and repositions Veterans/Members as directed. Follows instructions to use simple procedures and devices to strengthen Veterans'/Members' limbs and maintain flexibility.
  • Assists nursing staff with simple medical procedures such as suppositories, enemas, treatments, post surgical care and post-mortem care.
  • Always utilizes universal precautions. May use a variety of medical equipment (e.g., lifts, oxygen, geri- chairs, braces, prosthetic devices, catheter care, etc.) in accordance with physician orders, licensed nurse direction and/or care plan.
  • Assists in monitoring physical and mental status of Veterans/Members by utilizing appropriate medical equipment and techniques to take vital signs (temperature, pulse, respirations); obtain urine, sputum or stool samples; and track input/output.
  • Weighs Veterans/Members per licensed nurse instruction. Communicates and documents all information as appropriate.
  • Is responsible for medical record and worksheet documentation on all assigned Veterans/Members. Keeps records and prepares simple reports.
  • Transports Veterans/Members to the dining room. Informs them about menu choices available for the day. Informs dining staff about
  • Veterans'/Members' choices and picks up food from dining staff. Serves Veterans/ Members their meals in courses; assists them if necessary.
  • Clears soiled items from table after each course and places them in assigned location. Maintains record of Veteran/Member food consumption.
  • Provides for resident care and comfort by responding to patient requests (verbal or call light) for assistance timely in accordance with the Resident Bill of Rights.
  • Keeps areas neat and sanitary by changing bedding and linens; cleaning and disinfecting commodes, urinals, bedpans and humidifiers; providing fresh water, etc. as assigned.
  • Listens to and communicates resident requests/concerns as appropriate.
  • Responds appropriately to licensed nurses and assists with assaultive or combative residents following established policies and procedures.
  • Reports to and assists licensed nurses in emergency situations rapidly and appropriately following established policies and procedures.
  • May perform other related duties such as transporting Veterans/Members to appointments outside the facility; transporting them to activities within the facility; assisting them in packing and moving their belongings or assigned furniture; and labeling their personal property.
  • May perform clerical duties (e.g., answering phones, filing, etc.) as needed. Completes special projects as assigned.
  • Actively involved in AMS processes such Huddle Boards, Problem Solving, Job Breakdown Sheets, Success Tracking, and Standardized Work.
  • Participates fully in the QAPI process, our Quality Assurance and Performance Improvement program mandate by CMS which incorporates AMS.
Knowledge, Skills & Abilities (KSAs):

Knowledge of:

  • Principles, practices and techniques of para-professional nursing care, treatment and services appropriate to the work setting
  • State and agency laws and regulations governing para-professional nursing practices
  • Practices and Procedures of using medical equipment in assigned duties
  • Practices and procedures established for the work environment
  • Practices and techniques applied in maintaining a safe, orderly, therapeutic and hygienic environment

Skill in:

  • Applying methods and techniques of para-professional nursing practices
  • Applying and interpreting State and agency laws and regulations governing para-professional nursing practices

  • Applying methods and techniques in maintaining a safe, orderly, therapeutic and hygienic environment

  • providers.
Selective Preference(s):
  • One or more years of Skilled Nursing Facility or Long-Term Care Experience preferred.
Pre-Employment Requirements:
  • Current Arizona State Certified Nursing Assistant License
  • Current Negative TB skin test (Mantoux skin test) within the last 6 months or a written statement from a physician, physician's assistant or a registered nurse practitioner indicating freedom from tuberculosis, if in the past has had a positive skin test or tuberculosis
  • Currently possess or ability to obtain and /or retain an Arizona Department of Public Safety Level One Fingerprint Card
  • Current CPR card that was obtained by physical assessment
