Description:Principal Duties and Responsibilities (Essential Functions):

  • Works under general direction and supervision of charge nurse.
  • Provides bedside services.
  • Works under close supervision with awareness that error will result in disciplinary action.
  • Carries out duties and responsibilities in conformance with established routine.
  • Assists with physical comfort and mental well-being of resident’s complete daily requirements and tasks in care and treatment such as bathing, feeding, dressing observing intake of food, care of hair, nails.
  • Detects and corrects situations that have a high probability of causing accidents or injuries to residents.
  • Observes and reports symptoms, reactions and changes to Charge Nurse.
  • Encourages and participates in rehabilitative measures.
  • Obtains clarification from the Charge Nurse, when any questions regarding interpretations of assignments or functions come about.
  • Provides patient care services according to standard procedures and techniques and as per facility policies.
  • Properly uses equipment and supplies, and ensures the cleanliness and storage of these items.
  • Teaches, encourages, and maintains resident’s personal hygiene in regard to company policies and procedures and the facility’s established methods and techniques.
  • Monitors, assists, or completes baths.
  • Monitors, assists, or completes appropriate dress.
  • Monitors, assists, or completes trimming of fingernails and toenails, combing and shampooing of hair, and shaving of male patients-residents.
  • Monitors, assists, or completes oral hygiene including proper care of dentures.
  • Gives oral hygiene to helpless or comatose residents.
  • Monitors, assists, or completes bed making as assigned.
  • Maintains an awareness of facility’s method for handling clean and dirty linen.
  • Encourages and assists, when necessary, with ambulation of resident.
  • Demonstrates knowledge of each individual’s care plan and performs tasks as required by this plan.
  • Demonstrates knowledge of certain diagnosis and their limitations, i.e., diabetic: no sugar, caution in cutting nails.
  • Monitors or assists residents at meal time with eating or in using proper table manners and reports changes in appetite to Charge Nurse.
  • Assists or teaches residents, under general supervision, to do active and passive exercises.
  • Assists or teaches residents in using braces, wheelchairs, canes, crutches, self-help devices, walkers or other special equipment.
  • Assists residents in maintaining an orderly and clean environment.
  • Demonstrates knowledge of bowel and bladder training programs or other special programs for individual residents, and follows guidelines as set forth by policies and procedures of interdisciplinary team members.
  • Observes, recognizes, and reports all changes in patient-resident behavior, actions or physical conditions to Charge Nurse.
  • Maintains a cheerful attitude and treats all residents with respect and kindness.
  • Reports any accident/incident immediately to the Charge Nurse.
  • Maintains information concerning patients-residents confidentiality.
  • Completes and adheres to nursing assignments according to instructions and guidance provided by the charge nurse.
  • Supports the facility and the management’s philosophy, objectives, policies and procedures.
  • Reports personnel or patient-resident care problems to the Charge Nurse.
  • Attends and participates in orientation and in-service training programs.
  • Assumes responsibility for growth and development through observations, on-the-job training and workshops.
  • Functions as a team member to perform and to maintain all standards of care
  • Answers call light devices promptly.
  • Demonstrates at all times appropriate conduct, behavior and appearance according to company policies.
  • Uses proper techniques in giving bedpans and urinals, and in maintaining cleanliness and proper storage of these items.
  • Monitors intake and output as directed.
  • Turns and repositions residents per facility policy.
  • Assists in maintaining a clean patient environment by cleaning over bed tables and by assisting when necessary in general straightening of patient’s rooms.
  • Provides fresh water to residents and keeps water pitchers and glasses clean.
  • Performs all activities of daily living, according to treatment plan, such as bathing, feeding, etc., for the department residents.
  • Bathes and changes the clothing of residents after incontinent episodes.
  • Demonstrates basic knowledge of infection control techniques.
  • Participates in all facility’s Staff Development Program.
  • Performs all other duties as assigned in order to meet Federal and State standards, and complies with the philosophy, objectives, policies, and procedures of McCullough Hall Nursing Center.

Requirements:Qualifications:

  • Must perform only services of a type for which he/she has demonstrated competence.
  • Must have a personality attuned to the requirements of meeting needs of the infirm and aged.
  • Must demonstrate competency in skills and techniques necessary to care for residents’ needs as identified through Resident Assessment and described in the Plan of Care.
  • B.F.O.Q. - Female Only (Bonafide Occupational Qualification)
  • Must have knowledge of facility’s policies and procedures and carry these through.
  • Must have knowledge of state and federal requirements.
  • Must have familiarity with functions of various departments.
  • Must have understanding of standard techniques in personal service care of residents and care of equipment, including mechanical lifting devices and other handling aids, and supplies.
  • Must be able to use initiative and good judgment.

Education and Experience: Must be a graduate of state-approved certified nurse aide school.
Licensure/Certification: Must have successfully completed state board of certification and carry a current State of Texas certification as a certified nurse aide.
Working Conditions:

  • Must be able to stand and walk throughout shift on tile floors and carpeted surfaced in making rounds, supervising staff and working with residents in their rooms.
  • Must be able to sit up to two to three hours a day working on reports, charting and other duties.
  • Must be able to lift 45 pounds.
  • Must be able to assist in pushing wheelchairs, geri-chairs, and carts with supplies throughout an eight-hour shift.
  • Must be able to bend and twist when assisting and transporting residents and bending over to pick up items from floor.
  • Must be able to properly handle blood and urine.
  • Must have corrected vision to be able to read care plans, physicians’ orders, review nurse’s notes, and a variety of other written material. Must also be able to see various degrees of color on resident’s skin as well as read medications correctly.
  • Must be able to have corrected hearing to be able to hear residents and staff conversations and be able to communicate frequently throughout the day in discharging various orders to staff.
  • Overall environment: Must be able to work in setting where floors are often wet as well as be able to work around odors as well as oxygen. May be occupationally exposed to blood and other potential infectious materials. Must be able to comply with infection control procedures.
  • Must not be short-tempered and must be slow to anger. Must be able to respond in a calm demeanor and be able to work in an often-stressful situation when dealing with residents, families, and employees who may be abusive or threatening.
  • Must have a warmth, understanding and responsiveness to residents and their demands and reactions.
  • Must be able to deal with positive feelings for families and visitors, exercising patience and tact.
  • Must be able to use initiative, judgment and resourcefulness to make decisions.
  • Must be alert and able to identify hazards, should they occur, in order to avoid them by reporting immediately or if directed to take corrective measures.
