Job description

Kinderhafen Partners LLC is a locally owned and operated duo of group homes, working hard to serve up to ten medically fragile children and their families. The mission is to provide individualized, evidence-based health care in a home setting. This allows the staff to respect and support the role of the family on a deeper level, all the while providing advocacy, care management services and education for the caregivers and family alike.

Our staffing ratios allow for wholistic care for the pediatric population, serving with one nurse and one CNA for up to five children, with nurse on call for emergencies. Experience with tracheostomies and ventilators preferred, but we will invest training into willing candidates.

JOB TITLE: Certified Nursing Assistant

REPORTS TO: Licenced Practical Nurse, Registered Nurses, Case Managers, Internal Resource Coordinator, Clinical Nursing Supervisor, and Management Team

JOB SUMMARY: As a Certified Nursing Assistant CNA, you will have the unique opportunity to positively impact our residents every day. You will partner with nurses and other healthcare staff to provide the patient’s necessary medical care. In addition to providing the necessary care, you will assist residents with their daily healthcare, personal needs, safety and comfort.

The regular population served by Kinderhafen is 0-18yrs, however it is possible that person’s older than 18 could be in residency.

The staff member must demonstrate and maintain respectful and professional conduct within the Kinderhafen Partners Organization, as well as with outside customers and vendors.

The staff member must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served. The individual must demonstrate knowledge of developmental stages, possess the ability to assess the patient’s age-specific needs, and provide the care needed as described in Kinderhafen Policies and Procedures, and within the confines of their credentials.

ESSENTIAL FUNCTIONS:

  • Motivate and assist residents with their daily living activities while providing a positive physical, social and psychological environment
  • Ensure residents receive proper personalized care as needed
  • Assist with nursing functions, including taking vitals, weight and measurement
  • Prepare residents for meals and assist with feeding and meal preparation as needed
  • Communicate with nurses, residents and family members
  • Ensure proper healthcare documentation of resident charts
  • Laundry and housekeeping
  • May at times be required to travel while assisting nurses and residents to appointments and outings.
  • Encourages residents to participate in prescribed therapies, dietary regimen and activity programs, as appropriate.
  • Competently demonstrates and is able to support nurses during approved safety and emergency procedures.

  • Demonstrates initiative and a “see something, say something, do something” attitude to promote honesty, safety, and respect within the work environment.
  • Collaborate with staff, supervisors, and management team to address and promote desirable attitudes and actions in the homes that align with the Kinderhafen Partners culture and mission.
  • Maintain the safety and cleanliness of the homes either directly or via communication with the management team.
  • Communicates changes in resident’s status or condition to appropriate nursing staff. Provide a detailed hand-off report to the oncoming relief CNA.
  • Implements and executes the plan of care by completing Nursing Assistant interventions/orders.

SECONDARY FUNCTIONS:

  • Offers advocacy and emotional support to resident’s families, e.g., during separation adjustment, the grief process, coping with chronic illness and working with the cognitively impaired resident and his/her family.
  • Organizes work day according to accepted routines and prioritizes activities.
  • Participates in company in-service programs by attending scheduled in-services, suggesting in-service topics, teaching other staff on an informal basis, as required, and by attending approved outside in-service programs and sharing information received with staff.
  • Participates in company meetings, programs, staff meetings, and community outreach programs.
  • Maintains a safe and comfortable environment for residents, staff and visitors.
  • May be required to perform other related duties for which the individual is qualified.

EDUCATION: Completion of State approved Nursing Assistant Program and Current Certification in Washington State, and have a high school diploma/transcript or equivalent.

EXPERIENCE: Prior experience with pediatric patients is helpful. CPR certification as per AHA guidelines at the health care provider level. Food handler’s certification.

SKILLS: Interest in and understanding of the special needs of the pediatric client. Has well developed written and verbal communication skills in order to interpret written instructions, communicate and document effectively. Has well-developed motor coordination and manual dexterity in order to perform physical skills/activities. Able to prioritize appropriately and make sound judgements. Maintains confidentiality, professionalism, and quality customer service.

WORKING CONDITIONS: Works in clean, heated and air conditioned areas. Exposure to unpleasant sights, smells, infectious diseases, and combative or rude patients/family members is very possible. Lighting may be kept low at times to provide a low stimulation environment while residents rest. Actual hours worked will be dependent on needs and current staffing patterns. Holiday and weekend coverage is divided among nursing staff. Shift rotation to all shifts may be required although efforts will be made to accommodate needs and preferences.

PHYSICAL DEMANDS: Physical strength, endurance and motor coordination is necessary for long periods of walking, standing, performing physical activities or skills and assisting with lifting, transferring and repositioning residents. Must have good vision, hearing, speech and language abilities in order to perform and communicate nursing activities.

EXPOSURE CATEGORY: Tasks that involve exposure to blood, body fluids, tissues, or hazardous materials. This includes all procedures or job related tasks that involve inherent potential for mucous membrane or skin contact with blood, body fluids or tissues, hazardous materials or potential spills or splashes of them.

*Base wages are based off years of experience, expertise and certifications.
