The Montclair is a Assisted Living Home in Decatur, Ga, part of the Clairmont Place Community. We are tucked away on 16+ acres of land with a private lake and walking paths, in a park-like setting. We are conveniently located off Clairmont Rd, near Emory and the VA Hospital. The Montclair is owned by the Clairmont Place Community members and run under the direction of a board of resident representatives.


The Montclair offers a continued, higher level of support for our elders who are no longer able to live independently. Our focus and goal is person-centered care with a high level of engagement and a low resident to caregiver ratio, allowing our caregivers to focus and individualize their attention on elders and their care plans. Our management staff has a combined 30+ years of leadership experience. Montclair's team of care partners help residents perform tasks safely, prioritizing autonomy, engagement and independence. We care for the human spirit as well as the body.


The right person for this role will have the ability to focus individual care without sacrificing their vision for task completion for the day, the ability to work with a team to accomplish goals, the ability to communicate with care, consideration and professionalism with both elders, team mates and family members, and the ability to serve as a true partner in caregiving for our residents. You must have experience working with elders and heart for serving this population.


All caregivers must be CNA certified with current certificate. Clairmont Place requires new hires to be vaccinated against Covid - we serve a vulnerable population. New hires must be able to pass a drug and background screen, a physical and have a negative TB test. You must have current CPR certification.


This is a full time hourly position. The current schedule is a 12 Hour Shift, 7a - 7p and 7p - 7a. DUTIES AND RESPONSIBILITIES

  • Administer physician ordered medication to resident's and maintain related medical records under the supervision of the Clinical Director
  • Verify identity of residents receiving medication and record name of drug, dosage, route, and time of administration on electronic MAR
  • Presents medication to resident and observes administration of medication, using specified procedures
  • Takes vital signs or observes residents to detect response to specified types of medication and prepares report or notifies Clinical Director of unexpected reactions
  • Documents reasons prescribed drugs are not administered
  • Adheres to medication administration policy
  • May receive supply of ordered medications and assemble drugs for administration to resident
  • May record and restock medication inventory
  • Participates in an annual proxy caregiver skills competency check which meets the requirements contained in the standardized clinical skills competency checklist used to certify proxy caregivers.
  • Follow state proxy caregiver protocols for the community to maintain compliance


May perform the following tasks related to the administration of medications utilizing only unit or multidose packaging of medications:
  1. Administer physician ordered oral, ophthalmic, topical, otic, nasal, vaginal and rectal medications
  2. Administer insulin, epinephrine, and B12 pursuant to physical direction and protocol
  3. Administer medications via a metered dose inhaler
  4. Conduct finger stick blood glucose testing following established protocol
  5. Administer a commercially prepared disposable enema ordered by a physician
  6. Assist residents in the supervisor of self-administration of medication


Monitors and records temperature, pulse, respirations and blood pressure, as directed by the Clinical director.

• Report significant changes in the residents' condition log and med aide communication log

Change simple dressings and empty ostomy bags, Colostomy bags and catheter bags • Reminds and assists resident in taking pre-measured medications

Maintains an accurate and complete record of care provided and documents observations appropriately

• Responsible for maintenance of electronic medical records, assuring accuracy, completeness and compliance with regulations, certification standards, legal and ethical standards

Supports the resident, family, and proxy caregiver

• Provides bed bath, shower or tub bath as appropriate

• Provides appropriate care of skin, hair, nails, mouth and feet

• Provides good body alignment, positioning, and range of motion exercises for non-ambulatory residents • Assists resident in and out of bed and with ambulation

• Assists resident with use of bedpan, urinal, commode and bathroom

Assists with feeding and dressing resident

Provides personal care to the resident in a compassionate and understanding manner consistent with the Eden philosophy • Maintains the dignity of the resident

Provides person-centered care to improve the quality of life

Consults and collaborates with the management team and others involved in the resident's care

• Provides leadership oversight of team members and accountability to policies and procedures such as nametags, gait belts, clinical standups and professional interactions

Assists residents during activity programs and mealtimes

Focuses on solving conflict and not blaming

• Listens to others without interrupting and keeps emotions under control- Remains open to others' ideas and tries new things






Requirements:

QUALIFICATIONS

  • High school diploma or GED
  • Current Georgia Med Tech License
  • Must successfully clear a background check/ fingerprinting, TB skin test or x-ray, and health screening, drug screening
  • Must be fully vaccinated against Covid- First two shots mandatory for all Clairmont Place employees
  • Current CPR certification
