Clinical Transplant Coordinator (RN)

A recognized healthcare organization in New York City is currently seeking a licensed Registered Nurse (RN) to join their their staff in the Bronx as a Clinical Transplant Coordinator.
About the Opportunity:

• Schedule: Monday to Friday
• Hours: 7:30am to 6:30pm
• Department: Transplant Surgery

Responsibilities:
The Clinical Transplant Coordinator (RN) will be responsible for:

• Providing comprehensive care to transplant patients, across the health care continuum from inpatient to outpatient
• Providing healthcare education to patients that have received a transplant
• Performing comprehensive assessment to develop and implement treatment plans
• Monitoring the effectiveness of clinical interventions
• Providing ongoing support, counselling, and education to meet patient needs
• Incorporating quality assessment, performance improvement, clinical research and teaching to maximize and improve patient care activities
• ??Performing other duties, as needed

Qualifications:

• 4+ years of Clinical experience in a highly specialized complex Care program
• Bachelor's Degree in Nursing
• NYS Registered Nurse (RN) license

Desired Skills:

• Master's Degree in Nursing
• Transplant experience
