Job Description
SUMMARY: Provide direct services including individual, couple, family and group therapeutic services to patients including assessments, treatment plans and discharge planning.
FUNCTIONS OF POSITION:
1. Provide therapeutic services to patients and their families and provide psychosocial educational materials as deemed appropriate.
2. Work in collaboration with multidisciplinary team members including external partners to assess needs and coordinate services for patients and their families by providing evaluation/treatment in the form of individual, martial, family and group therapy.
3. Assess, evaluate and counsel patients to develop clinical treatment plan along with diagnosis of psychosocial disorders and completes required documentation and paperwork in a timely manner.
4. Coordinate services with other providers both internal and external.
5. Demonstrates the knowledge of and sensitivity to a variety of cultural and ethnic backgrounds.
6. Perform short term counseling and crisis intervention, as necessary.
7. Provide services within the scope of practice established by respective licensing Board.
8. Demonstrates an ability to set priorities in directing appropriate patient care, functions in an organized and time conscious manner and maintains complete accountability for actions taken.
9. Enhances professional growth and development through participation in education programs, current literature, in-service meetings, and workshops.
10. Maintain appropriate license by complete CE requirements and renewal of license.
11. Attends meetings as required and performs other related duties as assigned or requested.
12. Actively supports and promotes adherence to the policies and procedures of the Hospital and Social Services as outlined in policy and procedures.
QUALIFICATIONS:
EDUCATION: Masters of Social Work, counseling, psychology or related field; LCSW preferred.
CREDENTIALS: As above.
SPECIAL:
PATIENT CONTACT: Must have the knowledge and skills necessary to provide care appropriate to patients and individuals of all ages and varied cultural and ethnic backgrounds on a daily basis.
