Constellation is seeking an LPN for a Clinical Team Assistant Position for our Hospice program.

Responsibilities:
- Support for Hospice Clinical Supervisors:
- Assist Hospice Clinical Supervisors in both administrative and clinical tasks to ensure smooth operation.
- Task Management:
- Help manage scheduling, productivity, team assignments, and visit utilization effectively.
- Triage calls from staff, patients, and family members to provide timely and appropriate responses.
- Record Maintenance:
- Maintain accurate clinical records, statistics, and reports.
- Manage case conferences, Interdisciplinary Team (IDT) notes, as well as complaint and incident logs.
- Clinical Documentation System:
- Collaborate in establishing, monitoring, and maintaining the clinical documentation system.
- Ensure the documentation system aligns with clinical, quality, and regulatory requirements.
- Electronic Record Management:
- Assure the availability of all required documentation in the electronic record, adhering to agency policy.
- Adherence to Policies:
- Ensure compliance with agency policies and relevant regulations in all responsibilities.
- Flexibility:
- Perform other duties as assigned, demonstrating flexibility and adaptability.
Requirements:
- Licensed Practical Nurse in the State of Connecticut.
- Previous Home Care and/or Hospice experience is preferred.
- Excellent time management, organizational, customer service and technical skills.
