Job description

Constellation is seeking an LPN for a Clinical Team Assistant Position for our Hospice program.

Responsibilities:

  • Support for Hospice Clinical Supervisors:
    • Assist Hospice Clinical Supervisors in both administrative and clinical tasks to ensure smooth operation.
  • Task Management:
    • Help manage scheduling, productivity, team assignments, and visit utilization effectively.
    • Triage calls from staff, patients, and family members to provide timely and appropriate responses.
  • Record Maintenance:
    • Maintain accurate clinical records, statistics, and reports.
    • Manage case conferences, Interdisciplinary Team (IDT) notes, as well as complaint and incident logs.
  • Clinical Documentation System:
    • Collaborate in establishing, monitoring, and maintaining the clinical documentation system.
    • Ensure the documentation system aligns with clinical, quality, and regulatory requirements.
  • Electronic Record Management:
    • Assure the availability of all required documentation in the electronic record, adhering to agency policy.
  • Adherence to Policies:
    • Ensure compliance with agency policies and relevant regulations in all responsibilities.
  • Flexibility:
    • Perform other duties as assigned, demonstrating flexibility and adaptability.

Requirements:
  • Licensed Practical Nurse in the State of Connecticut.
  • Previous Home Care and/or Hospice experience is preferred.
  • Excellent time management, organizational, customer service and technical skills.
