Position Highlights:
  • Position: Clinical Specialist Pharmacy, Glenbrook
  • Location: Hybrid position: Office and Rotating Medical Group practices
  • Full Time/Part Time: Full Time
  • Hours: 40 hours per week
  • Required Travel: Yes. May need to travel to other Endeavor Health sites if applicable
Join an established team of clinical pharmacists supporting Population Health/Ambulatory Care! You will serve as a member of the interdisciplinary care team to address generic prescribing, medication adherence and appropriate statin prescribing and provide patient care support including consulting with physicians and patients on medication therapy.
What you will need:
  • License: Current Illinois Registered Pharmacist
  • Education: PharmD degree from an accredited school of pharmacy. Completion of a PGY1 and/or PGY2 pharmacy residency is preferred.
  • Certification: Board certification or equivalent experience preferred
  • Experience: At least 3 years of experience as a clinical pharmacist working in Population Health/Ambulatory Care in a corporate and/or clinic setting.
  • Skills: Prior EPIC experience preferred, Excel (creating and analyzing spreadsheets/reports, PowerPoint
What you will do:
Patient Care
  • May perform Comprehensive Medication Reviews, Collaborative Drug Therapy Management, and other MTM services for high-risk patients referred to the team.
  • Consult with prescribers on medication initiation, dosing, titration, monitoring, and patient education.
  • Perform medication reconciliation to ensure patient’s medication list is as accurate as possible, particularly during transitions of care, as needed.
  • Recommend appropriate lab tests to ensure adequate monitoring of medication therapies.
  • Provide recommendations to help patients manage their out-of-pocket costs for medications.
  • Provide patient education around medications, devices, and disease states.
Quality Improvement
  • Analyze EPIC and take action on monthly or more frequent commercial and governmental payer and internally developed reports focusing on improving generic prescribing; medication adherence for statins (MAC), diabetes (MAD), and hypertension (MAH); and statin prescribing for patients with cardiovascular disease (SPC) and diabetes (SUPD).
  • Participate in projects to improve the medication use process within the health system
  • Provide formal and informal educational sessions for prescribers and other staff to improve their medication-related knowledge.
  • Assist in developing and revising standards of care for the Population Health/Ambulatory Pharmacy program using evidence-based medicine.
  • Collaborate with clinic staff to improve workflows and address site-specific needs
  • Participate in appropriate committees and meetings within the health system.
