Behavioral Health Provider

Permanent, Full-time / Part-time positions in Portland, OR  

 

As the Behavioral Health Provider (BHP), you are a part of the primary care treatment team that identifies, triages, and manages patients with medical and behavioral health needs within the primary care setting. 

 

In addition, the Behavioral Health Provider will provide skill training through psycho-education and patient education strategies and will develop specific behavioral change plans for patients and behavioral health protocols for target populations. Working with the care team in situations in which a patient may be experiencing a behavioral health crisis and take the lead to coordinate support and stabilization for the patient as well as continued monitoring of the patients presenting behavior. 

 

The position requires Oregon licensure as an Licensed Clinical Social Worker (LCSW), or Psychologist as the position is located in the East County Primary Care Clinic, which is a covered Federally Qualified Health Center (FQHC) under the Bureau of Primary Health Care and the Centers for Medicare and Medicaid Services of the United States Department of Health and Human Services. 

 

As the Behavioral Healthcare Provider, you will provide

Medical Care

  • Provide advanced human services clinical assessment, analysis and/or treatment services involving complex psycho-social factors, biopsychosocial assessment and evidenced-based psychotherapy interventions to patients across the lifespan with medical conditions to support their management of recovery from these conditions. Inquire of current and past events, preference of care plan delivery, partnering with staff at the client level for a team approach, document decisions and next steps in the care process. 
  • Applies knowledge and experience in the relationship between medical and behavioral health aspects of health and disease while engaging in consultation with patients and primary care team members. 

Mental Health Care

  • Provide mental health assessment, diagnosis, and brief evidenced-based psychotherapy usually lasting from 1-6 visits per episode of care to Primary Care patients with mental health and addictions problems across the lifespan. 
  • Work with client to set up a care plan and how to continue the plan even as visits may or may not continue with the Behavioral Health Consultant. Continuity care is more frequent and continuous care to a small number of high needs patients in the population. 
  • Will be expected to assess suicidality and follow up with safety planning and other interventions to reduce risk and increase stability of patient.

Consultations

  • Provide behavioral health consultation and education to the Primary Care Provider (PCP) teams regarding psychosocial treatments. 
  • Partner with other providers and staff for each client and his/her/their needs to include family members and any other support systems for the client. 
  • Provides timely and succinct feedback to other primary care providers regarding consultation findings and recommendations. 
  • Formulates diagnostic and treatment recommendations and presents findings to treatment teams. 
  • Teaches primary care team members about behavioral health care topics and clinical issues. 
  • Read current journals and publications to keep current on consultation and care trends to include culturally based care, community interactions and connections, etc. 

Care Management:

  • Provide case management services to patients receiving behavioral health services, particularly in terms of coordinating care with behavioral health community partners. 
  • Maintain up to date notes and files per client event and interaction. 
  • Work closely with primary care team's community health worker to address social determinants of health.

Minimum Qualifications/Transferable Skills*:

Any combination of experience and training that would likely provide the knowledge and abilities is qualifying. A typical way to obtain the knowledge and abilities would be as noted below. All non-traditional education, training and experience will be evaluated.

 

Education/Training/ Experience: 

  • Equivalent to a Master's degree in social work, psychology or counseling
  • Two (2) years of professional work experience as a counselor, mental health treatment professional, Social Worker or therapist
  • Licensed Clinical Social Worker (Oregon Board of Licensed Social Workers), Licensed Psychologist ( Oregon Board of Psychologist) 

Other: 

  • Depending on assignment, candidates may be required to pass a criminal background check including fingerprinting. 
  • Valid Driver's License

  • Training and background at the post-master's level in primary care behavioral health 
  • Bilingual in Spanish and English 
  • Ability to effectively respond to crisis situations
  • Knowledge of or capacity to learn about the local behavioral health treatment system and community resources
  • Professional demeanor and interpersonal communication skills
