Job description

The Department of Mental Health (DMH) is seeking an experienced clinical professional to serve as the Clinical Risk Mitigation Coordinator for the Massachusetts Mental Health Center (MMHC).  The selected candidate will assist the Medical Director in providing site coordination of DMH Risk Management procedures and provide clinical consultation for safety and crises protocols for MMHC clinics and community services teams.

 

 

Duties and Responsibilities (These duties are a general summary and are not all inclusive):

  • Responsible for the site coordination of DMH risk management procedures.  Processes critical incident reports and complaints and provides the necessary administrative follow-up. Interfaces with the Area Office. Responds to queries, provides information, etc.
  • Identifies and tracks the critical incidents requiring clinical oversight. 
  • Provides consultation with programs and staff regarding follow-up and arranges risk review meetings as needed.
  • Facilitates the site's weekly Risk Review meetings. 
  • Provides consultations to cases presented in this forum and other client related meetings. Tracks the risk levels and recommendations for risk management of all risk assessed clients.
  • Maintains the site Risk Management Database and prepares and presents client information and updates at monthly Area Risk Database meetings.
  • Creates and maintains a system of coordinating the completion of annual risk assessments on all active site clients who meet the DMH risk management criteria. 
  • Collaborates with provider agencies regarding adherence to risk management policy and procedures.
  • Oversees creation of safety and crisis protocols and provides clinical consultation to the site Case Management staff.
  • Provides site representation and/or consultation in client care meetings/forums as needed.
  • Prepare/maintain administrative, statistical, and narrative reports as needed.
  • Represent the Center Director and the site at risk related meeting with the metro Boston area office, DMH central office and with vendors.
  • Responsible for conducting 10-day fact findings and other investigations.
  • Develops and conducts trainings for staff on screenings, safety plans, and evidence-based tools.
  • Provides clinical oversight to service authorization and conducts Needs and Means interviews for high-risk clients.
  • Perform other duties as assigned.

Required Qualifications:

  • Knowledge of the principles of physiological, psychological, and emotional growth and development.
  • Knowledge of and uses of diagnostic procedures employed.
  • Ability to motivate clients to participate in individual treatment plans
  • Knowledge of new therapies, interventions, and behavioral techniques
  • Experience providing outpatient care to persons with severe mental illness
  • Ability to participate in a multi-disciplinary treatment team.
  • Ability to understand and apply rules, regulations and laws governing agency and unit operations.

Preferred Qualifications:

  • Knowledge of available community resources, both public and private, including but not limited to, residential, rehabilitative, vocational, educational, and treatment options for the target population.
  • Knowledge of the laws, regulations, policies, and procedures governing the program and the Department as a whole, and entitlement programs.   

 

     

Please Note: This role is a 40 hour per week position with a  Monday through Friday schedule starting daily at 8:30 am until 5 pm.

MINIMUM ENTRANCE REQUIREMENTS: 
REQUIRED WORK EXPERIENCE: 
At least two years of full-time, or equivalent part-time, professional experience as a clinical social worker after earning a Master's degree in social work.  
SUBSTITUTIONS: 
 -  A Doctorate in social work, psychology, sociology, counseling, counseling education, or human services may be substituted for the required experience on the basis of two years of education for one year of experience.  
-  One year of education equals 30 semester hours.  Education toward a degree will be prorated on the basis of the proportion of the requirements actually completed.  
REQUIRED EDUCATION: 
A Master's or higher degree in social work is required. 
LICENSES: 
-  Licensure as a Licensed Certified Social Worker or Licensed Independent Clinical Social Worker by the Massachusetts Board of Registration in Social Work is required.  
-  Based on assignment, travel may be required.  Those employees who elect to use a motor vehicle for travel must have a current and valid Massachusetts Motor Vehicle Operator's license or the equivalent from another state.
