Scope

The Clinical Resource Nurse (CRN) plays several vital roles in consulting with facilities in the improvement to the quality of care and quality of life and providing training and guidance toward obtaining overall success in their clinical operations, including by assisting to develop internal processes and implement systems. The Clinical Resource Nurse’s number one priority is assisting facilities in improving quality care and quality of life for facility clients in its ongoing efforts toward successfully achieving a CMS 5 Star rating.


Specific Duties

  • Provide consultation and training to assist facilities to maintain the standards, rules, and regulations by which each LTC facility is governed, through actively monitoring and verifying the following systems:

a. Accident prevention and management

b. Pressure injury prevention and management

c. Weight management

d. Psychotropic medication management

e. Physical restraints/devices

f. Pain management

g. Medication management

h. Bowel and bladder program, catheter/urostomy

i. Infection control program, immunization

j. Admission and discharge process

k. Change of condition

l. RNA program

m. ADL decline, contractures

n. Rehab services

o. Special Care i.e. dialysis, hospice, pacemaker, IV/TPN, chemo/radiation, respiratory, GT/NGT feeding, suctioning, assistive devices, ostomy

p. Special needs i.e. communication, vision, hearing

q. Unusual Occurrence

r. Staff development

s. Activities

t. Disaster Preparedness


2. The Clinical Resource Nurse will conduct quality rounds when at the facility including the medication room, utility room, central supply room, etc. The CRN will meet with residents and get to know their current condition. The CRN will review resident’s charts for completion and accuracy.

  • The Clinical Resource Nurse will conduct in-service/education from the regulations, facility quality indicators, past surveys/complaint visit, etc.
  • The Clinical Resource Nurse will review the Survey 2567, track any trends or repeated deficiencies. If a trend is identified, the Clinical Resource Nurse will provide hands-on education and training to the facility staff.
  • The Clinical Resource Nurse will assist the facility to prepare for the survey and should be available to assist during the survey.
  • The Clinical Resource Nurse will build professional relationships with the regulatory agencies i.e. CDPH. CMS, Public Health, OSHA and etc
  • The Clinical Resource Nurse will review consultant reports, i.e. Pharmacy, RD, Wound consults, Medical Records, etc.
  • Other assigned tasks as needed.

Reporting

The Clinical Resource Nurse reports to the Chief Clinical Officer on a daily, weekly and monthly basis to track and trend care issues and repeated deficiencies. The Clinical Resource Nurse submits the daily audit to the client facility Administrators and DON, Chief Clinical Officer as part of the client facility’s Quality Assurance Performance Improvement Program.

Maintaining Confidentiality

The Clinical Resource Nurse upholds privacy and confidentiality in handling clinical records in compliance with HIPAA.

Compliance As a Condition of Employment

Agreement to abide by all standards, policies, and procedures of the facility, including the facility’s compliance and ethics program, is a condition of employment. Compliance will be a factor in evaluating job performance. Violations, including failure to report violations, will result in disciplinary action, up to and including termination.
